  • Exercise mature, professional judgment and flexible in handling a wide range of interventions provided through social work, substance abuse and mental health. Makes independent behavioral health recommendations for isolated and vulnerable patients who have complicated and conflicting needs regarding medical treatment plan and implementing services to patients and families during the critical and adjustment phases of physical illness, disabilities and overcoming dependency needs.
  • Identifies and handles of patients in psychiatric emergency situation, assessment and management of suicidal, homicidal and other violent patients. Provides or arranges for emergency mental health services in a variety of settings.
  • Serves as a case manager, conducts initial case assessments, develops individualized service plans that identifies programs ans services appropriate to meet patient's needs; access, coordinates/facilitates appropriate community and/or other services.
  • Makes independent professionals decisions and recommendations for hospital discharge of patients to home, alternative living, community based, contract care, rehabilitation or long term care. Meets with patients/families in all family conferencing, especially family meetings where critical and serious decisions that will impact the lives of patients/families.
  • Prepares patient referrals and coordinates services for patients with other professionals, community agencies/resources such as social services, education, law enforcement, judicial and health organizations.
  • Participates in child and elder protection multidisciplinary meetings and provides input on treatments and intervention plans to alleviate conditions lending to abuse/neglect of vulnerable patient groups. Prepares report regarding caseload for the purpose of evaluation of programs.

Qualifications

To qualify for this position, your resume must state sufficient experience and/or education, to perform the duties of the specific position for which you are applying.
Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; social). You will receive credit for all qualifying experience, including volunteer and part time experience. You must clearly identify the duties and responsibilities in each position held and the total number of hours per week.
BASIC REQUIREMENT(S):
Degree: Social Work. Must fulfill all of the requirements for the master's degree in social work in a school accredited by the Council on Social Work Education.
Master's degree with practicum: Applicants who complete all of the requirements for a master's degree in social work that included a practicum are eligible for GS-09.
In addition to the Basic Requirements, you must also meet the Minimum Qualifications stated below.
MINIMUM QUALIFICATIONS:
GS-11:
Three (3) years of progressively higher-level graduate education leading to a Ph.D. degree or Ph.D. degree or equivalent doctoral degree; or, one (1) year specialized experience equivalent to GS-09 level. Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Provide case management services for inpatients and outpatients that require hospitalizations or supportive community placements. Provide health education to patients, perform mental status evaluation on new admissions, referred patients and consultation.
SELECTIVE PLACEMENT FACTOR:
This position requires a current, unrestricted U.S. State license as a Licensed Master Social Worker (LMSW), a Licensed Clinical Social Worker (LCSW), or a Licensed Independent Social Worker (LISW). Please provide a copy of your license with your application.
