Summary

The Medical Instrument Technician (Echocardiography) works in the Cardiology section Indianapolis Richard L. Roudebush VA Medical Center. This is an OPEN CONTINUOUS ANNOUNCEMENT. The initial cut-off date for referral of eligible applications will be 01/20 /2023 and applicants will be referred on monthly basis after that.

Duties and responsibilities include, but are not limited to:

  • Identify and record anomalies indicative of disease, injury, or other medically significant condition from ultrasound imaging and simultaneous recordings of the Doppler.
  • Incorporate the causes and results of a variety of symptoms and conditions into a plan of ultrasonic diagnostic examination.
  • Change and develop sounding techniques to accommodate such variables as limited patient mobility, variation in physical condition or dimension of the patient, presence of prosthesis or foreign objects, and ultrasonic response of different body parts.
  • Produce diagnostic quality images and Doppler frequency recordings using M-mode, two-dimensional, and Doppler ultrasound.
  • Has the intimate knowledge of thoracic anatomy and physiology as related to cardiovascular function.
  • Assist the charge nurse, supervisor, and RN's with other duties as assigned.
  • Performs diagnostic quality images and Doppler frequency shift recordings using M-mode, 2-D, pulsed and continuous wave Doppler spectral display and color flow Doppler imaging.
  • Performs complex procedures such as cardiac studies of the 4 chambers of the heart and valves using sector scanning techniques to detect stenotic and incompetent valves, chamber enlargement, and pericardial effusion, thrombus and pressure gradients across obstructed valves.
  • Performs transesophageal echocardiograms dobutamine and exercise stress and 3D echocardiograms.
  • Ability to perform complementary ultrasonic examinations (including noninvasive) transthoracic echocardiography, Bubble studies and contrast echocardiography (invasive), stress echocardiography, and transesophageal echocardiography.
  • Adapts transducer positioning, instrument controls and examination techniques to the individual patient and to the problem to be evaluated.
  • Monitors patient's condition during procedures.
  • Manages and Triages inpatient procedure consults on a daily basis.
  • Ensures that inpatient exams are completed in a timely manner.
  • Accurately documents information in the medical record and any other clinical or administrative applications.
  • Manages Echocardiography images in Centricity and copy to CD.
  • Performs digital image management and acts as Super User for imaging storage system.
  • Assists billing and coding with interpretation of exams and follow-up on outstanding billing and coding issues.
  • Completes encounters on all ECHOs performed, using the correct CPT codes, diagnosis, Service connection status and other field as required and assists management in overall encounter practices in ECHO section.
  • Participates in faculty conferences, task forces, and committees as assigned
  • Communicates effectively and professionally with employees at varying grade levels, patients and their families, and customers from outside facilities.
  • Precepts new staff and cardiology trainees in their development of echocardiography imaging skills.
  • Serves as a consultant to other health care professionals related to ECHO procedures.
  • Develops, coordinates, or presents educational programs towards improving productivity, patient outcomes, and treatment modalities as needed and assigned.
  • Performs dys-synchrony evaluations and assist in the timing delays for Bi-Ventricular Pacing.
  • Assists with the scheduling clerk in determining appropriateness of consults, triage, and priority when needed, etc.

Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency: Medical Instrument Technicians must be proficient in spoken and written English.
  • Education. There are no specific educational requirements for this occupation. Education may be substituted for experience only at the GS-4 and GS-5 levels. See the grade requirements part of this standard for information regarding educational substitutions.
  • Licensure or Certification. Licensure or Certification is not required for this occupation; however, it is strongly desirable at GS-6 or above as evidence of possession of the essential knowledge, skills, and abilities. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grandfathering Provision. The following is the standard grandfathering policy for all Title 38 Hybrid qualification standards. Some of these provisions may not apply to this occupation. Please carefully review the qualification standard to determine the specific education and/or licensure/certification/ registration requirements that apply to this occupation.

May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

All persons employed in VHA in this occupation on the effective date of this qualification standard are considered to have met all qualification requirements for the title, series and grade held, including positive education and licensure/certification/registration that are part of the basic requirements of the occupation. For employees who do not meet all the basic requirements required in this standard, but who met the qualifications applicable to the position at the time they were appointed to it, the following provisions apply:

Such employees in an occupation that does not require a licensure/certification/registration, may be reassigned, promoted, or demoted within the occupation.

Grade Determinations: At least 1 year of experience comparable to the next lower grade level (GS-8), which is directly related to the duties of the position to be filled. One year of experience includes, but is not limited to: identify and record anomalies indicative of disease, injury, or other medically significant condition from ultrasound imaging and simultaneous recordings of the Doppler. They incorporate the causes and results of a variety of symptoms and conditions into a plan of ultrasonic diagnostic examination. They change and develop sounding techniques to accommodate such variables as limited patient mobility, variation in physical condition or dimension of the patient, presence of prosthesis or foreign objects, and ultrasonic response of different body parts. They produce diagnostic quality images and Doppler frequency recordings using M-mode, two-dimensional, and Doppler ultrasound, etc. Additionally, applicants must fully meet the Knowledge, Skills, and Abilities (KSAs) at that level. This would be experience which demonstrates possession of the knowledge required in order to provide all aspects of echocardiography services.

In addition, the candidate must demonstrate, in their resume, the following Knowledge, Skills & Abilities (KSAs) and the potential to acquire the assignment specific KSAs designated by an asterisk (*):

1. Knowledge of the more complex examination and treatment procedures and techniques in order to provide training and supervision in these procedures, to evaluate and initiate performance improvement projects, and to write policies, procedures, and protocols, that pertain to Echocardiography.
*2. Knowledge of JCAHO and other regulatory requirements.
3. Ability to communicate effectively and professionally with employees at varying grade levels.
*4. Ability to manage and supervise employees.
*5. Ability to provide, or provide for, staff development and training.

The full performance level of this vacancy is GS-09. The actual grade at which an applicant may be selected for this vacancy is a GS-09.

Physical Requirements: The incumbent must be able to travel to treatment areas with ease while pushing an Echo Machine. The incumbent must be able to stand for up to 8-10 hours while providing patient care. The incumbent must be able to lift up to 20 pounds of equipment, supplies and records. The incumbent must be able to manipulate the probes during the procedure, pushing against the patient's chest in order to obtain the appropriate images. Additionally, the incumbent will be required to transport patients on stretchers, beds and wheelchairs. There is the potential for extended periods of sitting at computer terminals. The incumbent is expected to use proper preventive ergonomic techniques to perform duties.

Receiving Service Credit or Earning Annual (Vacation) Leave: Federal Employees earn annual leave at a rate (4, 6 or 8 hours per pay period) which is based on the number of years they have served as a Federal employee.
