Help

Clinical Practice

  • The CPS has responsibility for providing pharmaceutical care to optimize patient response and resource utilization.- Is directly involved in the design, implementation, and monitoring of therapeutic drug plans to achieve definite outcomes through direct interactions with patients and providers in assigned practice areas.- Is actively involved in reviewing patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes as required.
  • Communicates findings with other providers and presents appropriate alternatives to current treatment plans as needed.- Independently initiates, modifies, or discontinues medication therapy for items other than controlled substances under a scope of practice.
  • Provides patient-specific drug therapy modifications to maximize patient response and minimize drug interactions, adverse drug effects, and polypharmacy while providing cost effective treatment.
  • Orders, performs, reviews, and analyzes appropriate laboratory tests and other diagnostic studies necessary to monitor and support the patient's drug therapy. Follows-up with patients on lab or test results to discuss the plan of therapy.
Education:
  • Assesses education needs of the patient and/or caretaker(s) to help ensure that pharmaceutical outcome goals are met.
  • Provides education using suitable oral or written communication techniques appropriate for the age, development and cultural requirements of the patient and/or caretaker(s).-
  • Provides drug information to other pharmacy staff and healthcare professionals.
  • Identifies, develops, and/or provides in-service training for other pharmacy staff or healthcare professionals.-
  • Serves as adjunct preceptor for undergraduate, graduate and post-graduate pharmacy students and pharmacy residents (as applicable).
  • Provides specialized education and training support about pharmacotherapeutics to other healthcare professionals on assigned rotation as applicable.-
  • Assists in the orientation and training of new pharmacy employees, students and volunteers.
  • Is responsible for maintaining continuing education required for licensure, as well as the competency and mandatory training required for a CPS at the Wilmington VAMC.
Administrative:
  • Is responsible for the daily operational status of the various aspects of the pharmacy programs. The CPS shall keep supervisors and other pharmacy staff members informed of administrative problems or episodes of clinical pertinence which may impact on the pharmacy service effectiveness or which may enhance staff performance.-
  • Helps ensure that all aspects of medication accountability, medication security, equipment maintenance and timeliness of service are in accordance with local medical center policies and other applicable VA, FDA, DEA, State and Federal regulations.-
  • Prioritizes work and makes administrative decisions when supervisory pharmacists are not available or in instances where immediate decisions are required. Solves problems, coordinates care, and organizes responsibilities to maximize outcomes in their area of clinical expertise.-
  • Prepares and provides written information in a professional manner with attention placed on spelling, grammar and organization. Effectively communicates orally and in writing to persuade and influence clinical and management decisions.
  • Is responsible for creation, development, implementation and ongoing monitoring of policies and procedures related to the CPS clinical area of expertise.
Quality Assurance and Improvement:
  • Assists with the Quality Assurance (QA) program within Pharmacy Service as it relates to clinical practice. Is responsible for maintaining QA data for their focus of clinical practice.
  • Participates in interdisciplinary care teams and committees to improve the quality of patient care.- Participates in the Medication Use Evaluation (MUE) process by assisting in the identification of problem areas, criteria development, analysis of findings, and recommendation of follow up actions (e.g. education) that are deemed necessary.-
  • Analyzes data and completes reports, including clinical pertinence reviews (peer reviews) in accordance to established facility policy. Prepares, submits, and presents reports for P&T council and other reports as assigned in a proactive, timely and accurate manner.
Work Schedule: Tour of duty between 6 Am and 6PM and may include nights, weekends, or holidays as needed.

Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Education: (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.)(2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test.
  • Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure.
    • Exception. Non-licensed pharmacists who otherwise meet the eligibility requirements may be given a temporary appointment at the entry level as a Graduate Pharmacist under the authority of 38 U.S.C. § 7405(c)(2)(B). The appointing official may waive the requirement of licensure for a period not to exceed 2 years for a pharmacist that provides care under the supervision of a licensed pharmacist. For grade levels above the GS-11, the candidate must be licensed.

  • English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).

Grade Determinations: GS-13a) Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level. Examples of GS-12 level equivalent experience are but not limited to: knowledge of professional pharmacy practice; ability to communicate orally and in writing to both patients and health care staff; knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security; and skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

Clinical Pharmacy Specialist: The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy.

In addition to meeting the GS-12 KSA the Pharmacists assigned to this position must demonstrate the following KSAs at the GS-13:
a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
b. Expert understanding of regulatory and quality standards for their program area.
c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
e. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

Physical Requirements: Moderate lifting, 15-44 lbs. Significant computer work/typing, prescription order entry and processing, contact of patients. Certain tasks are repetitive in nature and require multiple steps performed in succession for success. Incumbent must be able to stand or sit in one place for considerable time during completion of complex tasks. Complexity of mental processes involved is extensive, demanding, and challenging.
