The Health Care for Homeless Veterans (HCHV) authorizes the provision of outreach services, assessment, health care and referrals for VA/community services to eligible Veterans. The objective of the HCHV Programs is to re-integrate homeless Veterans with psychiatric and substance abuse disorders into the community and to help them achieve the maximum level of health and social functioning as possible. Contract services are furnished by existing community agencies with supplemental care and overall program administration provided by the VA. The HCHV Program Manager retains direct programmatic responsibility for all HCHV programs. This position reports directly to the Chief, Social Work Service, who provides guidance as needed.

The position supervises the Health Care for Homeless Veterans (HCHV) Programs. These programs include: Health Care for Homeless Veterans Outreach, Housing and Urban Development/VA Supported Housing (HUD/VASH), Contract Residential Program, Veterans Justice Outreach (VJO) Program, Substance Use Disorder for HCHV program, and Community Employment Coordination Program.

The Program Manager has significant personnel management responsibility for a team of social workers, counselors, and administrative staff who function within the programs under HCHV. Staff are located at the main facility as well as the community clinics and range from GS5 administrative to GS12 clinical. The Program Manager assists in planning, establishing, and implementing policies and procedures; reviewing and planning HCHV budget needs, developing individual or group goals and objectives as well as monitoring, operating, evaluating, coordinating, and overseeing the staff assigned to the program. The incumbent will ensure subordinate staff are aware of and adhere to current VA/VHA policy and meet the requirements of accrediting organizations and State licensing boards. The incumbent will conduct ongoing intensive reviews and appraisals to ensure the accomplishment of objectives and adherence to standards set by governing bodies. The HCHV Program Manager is expected to conduct an annual program needs assessment, identify gaps in services and make strategic plans to address these deficiencies. The Program Manager manages the annual Community Homelessness Assessment, Local Education and Networking Groups (CHALENG) project.

The Program Manager provides social work services at the Advance Practice Level to the homeless Veteran population and practices under the social work scope of practice. Services include provision of independent and expert clinical psychosocial case management, clinical treatment, advocacy, and coordination of linkages to other appropriate VA or community service providers and/or agencies as needed by the homeless client. This is accomplished in collaboration with the members of an
interdisciplinary treatment team. The incumbent will assist Veterans, as needed, in other clinics such as Primary Aligned Care Teams (PACT), inpatient Community Living Center (CLC), and Social Work Professionals in Housing and Urban Development VA - Supportive Housing (HUD-VASH), Domiciliary, and Medical Services.

Qualifications

Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency Requirement - Per VA Handbook 5005, Part II, Chapter 3, Section A, Paragraph 3j: No person will be appointed under authority of 38 U.S.C., chapter 73 or 74, to serve in a direct patient-care capacity in VHA who is not proficient in written and spoken English.
  • Physical Standards. See VA Directive and Handbook 5019.
  • Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
  • Independent Practice Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level.   • Advanced Practice Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series at the GS-12 level or higher in VHA must be licensed or certified by a state to independently practice social work at the advanced practice level. Preferred Experience:
  • 1. Experience in crisis intervention, case management, resources, community referrals and discharge planning.
  • 2. Knowledge and experience in program development, performance measures, and budgets.
  • 3. Knowledge and experience in the role of administration and management, addressing staffing and performance needs, productivity, and disciplinary action.
  • 4. Knowledge of addressing and assessing complicated psychosocial problems with those experiencing crisis to include provision of short-term, solution-focused counseling.
  • 5. Certification in Case Management is preferred, not required.
  • 6. Certification in Clinical Supervision is preferred, not required.
Grade Determinations: In addition to the basic requirements for employment, the following criteria must be met for the GS-13 grade level.
Experience: One year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidates may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.

In addition to the experience above, candidates must demonstrate all of the following KSAs:
(a) Skill in coordinating and implementing one or more specialty program(s), across the facility.
(b) Skill in local policy and practice development, procedures, and/or practice guidelines for the program as prescribed by the program handbook or national directive.
(c) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
(d) Ability to collaborate with internal and external partners to further program goals and enhance patient centered care.
(e) Ability to provide the full range of supervisory duties including assignment of work to be performed; performance evaluations; and selection of staff, training, and recommendation of awards, advancements, and when appropriate, disciplinary action.
(f) Ability to oversee the fiscal matters of the functions supervised (including fund controls, contracts, and equipment expenditures), forecast resource and equipment needs, and administer the allocated budget.
(g) Knowledge of organizational structure and impact on program operations.

The actual grade at which an applicant may be selected for this vacancy is GS-13.
