The Cheyenne VA Medical Center, Mental Health Service Line is currently recruiting for one (1) Social Work Program Coordinator, HUD VASH Coordinated Entry Specialist (CES)
You will be expected to drive for this position and must have a current unrestricted state issued drivers license.
The incumbent functions as the advanced practice Coordinated Entry Specialist (CES) Program Coordinator for Homeless/HUD-VASH Programs at CVAHCS. This position enables VA Homeless/HUD-VASH Programs to fully collaborate with the community, including Continuum of Care (CoC) and community partners, in the coordinated entry efforts in that community. The incumbent provides specialized services to high complexity Veterans in the outreach function, as well as functions as a subject matter expert on Coordinated Entry for the health care system.
The incumbent has experience that demonstrates possession of advanced practice skills and judgement in clinical
services for homeless Veterans. Specifically, the incumbent supports the VA Homeless/HUD-VASH Programs by providing continuity of care for homeless Veterans between VHA and community-based programs, as the Veteran transitions from homelessness into programs such as HCHV Contract Residential Housing (CRS),
Residential Rehabilitation and Treatment Programs (RRTPs), and Grant Per Diem Programs (GPD) as well as directly to permanent housing through the Housing and Urban Development-Veterans Affairs Supportive Housing (HUD-VASH) Program, the Supportive Services for Veteran Families (SSVF), or other forms of housing in the
community. This continuity of care also includes coordination with the local Suicide Prevention Coordinator(s) (SPC) to enhance suicide prevention amongst homeless Veterans encountered through community-based programs. The incumbent provides leadership in the community by actively participating in the local plans to end Veteran
homelessness as well as conducting and leading the coordination of outreach services to homeless Veterans.
The incumbent must be able to develop appropriate professional relationships with community stakeholders, and is responsible for communicating potential CoC policy or ability to coordinate housing and services for homeless Veterans with the CoC or other key partners. The incumbent will facilitate VA involvement in the CoC's Coordinated
Entry System (CES) and will be responsible for helping VA meet federal guidelines on CES, as described by regulations and guidance published by HUD.
The incumbent needs to coordinate their efforts with local planning and support currently provided by VA, HUD, and USICH technical assistance initiatives designed to support local planning and the creation of a CES. This involvement requires that the incumbent is both aware and involved in current CoC plans to meet these federal requirements.
The incumbent will also function as a liaison, participating in case conferencing and other pertinent community meetings, as needed. The incumbent is the VA liaison for coordinating outreach efforts with local community partners; including ensuring linkages to VA services and the local SPC for those Veterans who are experiencing literal street homelessness and are at high risk for suicide.
The MHSL at CVAHCS is comprised of 129 FTEE of interdisciplinary providers and program staff serving approximately 5,000 Veterans annually through programs including: Intensive Community Mental Health Recovery, Health Care for Homeless Veterans, Grant and Per Diem, Housing and Urban Development-VA Supportive
Housing, Veterans Justice Outreach, Vocational Rehabilitation Services, Intimate Partner Violence, Military Sexual Trauma, Psychosocial Residential Rehabilitation Treatment, Suicide Prevention, Geriatrics, Veterans Integration to Academic Leadership, Substance Abuse Treatment, Post Combat Trauma, Primary Care Mental Health Integration, and Peer Support. CVAHCS is a Level 3 facility with medical and mental health services campus located in Cheyenne WY, and at its four Community Based Outpatient Clinics (CBOCs) (Fort Collins, Loveland, Sidney, and Rawlins), as well as through its Mobile Telehealth services in Wheatland, Rawlins, Torrington, Sterling, and Laramie.
The incumbent has experience that demonstrates the advanced practice skill level in social work.
The incumbent must be licensed or certified at the advanced practice level and able to provide supervision for licensure. The incumbent has specialized knowledge of practice that can be differentiated from the independent practice Social Worker by the ability to expand clinical knowledge in the profession, administer a facility program,
provide consultation and guidance to colleagues, role model effective social work practice skills, teach or provide orientation to less experienced Social Workers, develop innovations in practice interventions, and provide clinical supervision for social work licensure.

Qualifications

Basic Requirements:
Citizenship. United States Citizen
Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to the CSWE website to verify if that social work degree meets the accreditation standards for a master of social work.
Licensure. Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found on the OHRM website. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level and must be able to provide supervision for licensure.
GS-12 Social Worker Program Coordinators. Program coordinators at the GS-12 grade are administratively responsible for a clinical program providing treatment to patients in a major specialty, i.e., Suicide Prevention. They may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. They manage the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget. Individuals assigned as social worker program coordinator must be licensed at the advanced practice level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. They may have certification or other post-master's degree training for a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. In addition, the candidate must demonstrate the professional KSAs shown below.
Demonstrated Knowledge, Skills, and Abilities
1.Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
2.Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
3.Ability to supervise multidisciplinary staff assigned to the program.
4.Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
5.Ability to provide training, orientation, and guidance within clinical practice.
