Summary

This position is located in the Primary Care Service at the James E. Van Zandt VA Medical Center, a secondary care facility with 30 acute beds & a 30 bed Nursing Home Care Unit, & three Community Based Outpatient Clinics. Responsibilities include the management of a panel of patients, supervisory duties of a physician assistant or certified registered nurse practitioner. The Primary Care Service provides full outpatient medical services to veteran patients, Tricare & ChampVA beneficiaries.

The incumbent of this position will be assigned to the Huntingdon CBOC.

This medical center services over 25,000 unique veterans with over 150,000 visits yearly and is rapidly expanding in scale and scope. The successful candidate will have overall responsibility for Primary Care of a panel of patients at the James E. Van Zandt VA Medical Center or one of its three CBOC's.

Areas of responsibilities include but are not limited the following:

  1. Links clinical improvement to operational efficiency for the service. The intensity, activity, and broad program mission generate numerous complex problems which require the Physician to have outstanding skills and ability in planning, organizing, and directing their teamlet (teamlet consists of an RN, Clerk, LPN or health tech) which effective a positive impact on the interdisciplinary service, the interdisciplinary professional, and the health care delivery system. Additional members of a primary care teams include, Social worker, dietitian and clinical pharmacist.
  2. Formulates long and short-range goals and objectives, treatment plans, and programs (Move, VIST, Spinal Cord Injury, etc.) for their patients. In formulating such plans, coordinates with the Care Manager and teamlet to integrate the medical center performance measures, monitors, and VISN objectives.
  3. Initiates delivery of clinical preventive services using "Medical Home" principles, such as evidence-based guidelines, systematic assessment of all health needs, non-face to face encounters, patient centered, patient empowerment, patient preferences, team approach, and quality improvement.
  4. Prepare for, provide, and follow-up on the clinical needs of Primary Care patients. Reviewing patient data, have discussions about the care with colleagues and specialists, reviewing medical literature, contacting the patient or family to discuss their concerns or needs, and collaborating with potential and actual non-VA, community-based health agencies and facilities on patient's behalf.
  5. Prevention, health promotion, and disease management.
  6. Comprehensive "first contact" primary care model with patient and family as the hub, team based approach, functioning at optimal level of competency, and coordination of care across the continuum.
  7. Participates in medical staff decisions affecting patient care services. This involves collaborating with colleagues in developing, implementing, and evaluating philosophy, objectives, policies, and standards essential for delivering optimal care to patients.
  8. Manage complex diagnostic problems and difficult clinical management issues.
  9. Follow the principles of Patient centered care:
    • Honor the patient's expectation of safe, high quality and accessible care.
    • Enhance the quality of therapeutic alliances
    • Solicit and respect the patient's values, preferences, and needs
    • Systematize the coordination, continuity, and coordination of care
    • Empower patients and families through information and education
    • Incorporate the nutritional, cultural, and nurturing aspects of care
    • Provide for physical comfort and pain management
    • Ensure emotional and spiritual support
    • Encourage the involvement of family and friends
    • Support and sustain a satisfied and engaged workforce
  10. Performs other directly related duties as required.

EDRP Statement:
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan reimbursement program. If selected for this position you must meet specific individual eligibility requirements, and submit your application within four months of appointment. The final award amount (up to $200,000) and eligibility period is determined by the VHA Education Loan Repayment Service office.

Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed.
  • Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.
  • Completion of residency training, or its equivalent, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification.
  • Proficiency in spoken and written English.
  • Physical requirements outlined below.
Preferred Experience: Board certified in Internal Medicine or Family practice is preferred

Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-2 Physician Qualification Standard. This can be found in the local Human Resources Office.

Physical Requirements: Heavy lifting (30 pounds and over); heavy carrying (30 pounds and over); straight pulling (2-4 hours); pushing (2-4 hours); reaching above shoulders (1-2 hours); use of fingers; both hands required; walking (4-6 hours); standing (4-6 hours); repeated bending (2-4 hours); both legs required; ability for rapid mental and muscular coordination simultaneously; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in one eye to 20/20 and 20/40 in the other; both eyes required; ability to distinguish basic colors and shades of color; hear (aid permitted); mental and emotional stability.
