Summary

The incumbent will develop an assessment of the Veteran in collaboration with the interdisciplinary treatment team, the Veteran, family members and significant others.
The incumbent demonstrates an understanding of the range of treatment and skills needed for all adult patients including geriatric patients related to their physical limitations, psychological needs and age associated illnesses and conditions.
The incumbent must have an understanding of psychosocial needs, the problems of advancing age and respect for the diversification of the aging Veteran population, being sensitive to human growth and development, age-appropriate needs and concerns of the geriatric Veteran, and be able to provide current and advanced techniques to the treatment of this Veteran population.
The incumbent manages a normative workload in his/her area, establishing priorities for timely service delivery to prevent hardship for patient/families and decrease delays in discharge from the hospital.
The incumbent will act as a consultant to other team/staff members and will provide consultation/education to medical residents on social work practice, VA policy and procedure.
The incumbent will review the progress notes from the other providers to accurately determine the strengths and limitations of each Veteran being referred for services.
The incumbent will provide individual, family and group counseling as appropriate.
Work Schedule: Monday - Friday: 8:00 a.m. - 4:30 p.m. The incumbent will participate in a rotation of on-call social workers if necessary, including modification of tour of duty to accommodate patient care needs on weekends; and other duties as assigned.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
Citizenship: Must be a United States Citizen.
English Language Proficiency: Must be proficient in spoken and written English.
Education: You must have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited.
Licensure: You must be licensed or certified by a state to independently practice social work at the master's degree level.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
GS-11 Grade Determinations:You must be licensed or certified in social work at the independent practice level with at least one (1) year of post-MSW degree experience in the field of health care social work. A doctoral degree in social work from a school of social work may be substituted for the required one year of professional social work experience in a clinical experience. Must submit copy of transcripts if applicable. In addition, the candidates must demonstrate the Knowledge Skills and Abilities (KSAs) listed below:
Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services.
Ability to independently assess the psychosocial functioning and needs of patients and their family members and to formulate and implement a treatment plan, identifying the patient's problems, strengths, weaknesses, coping skills and assistance needed, in collaboration with the patient, family and interdisciplinary treatment team.
Ability to independently conduct psychosocial assessments and provide psychosocial treatment to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds. This requires knowledge of human development and behavior (physical and psychological) and the differential influences of the environment, society and culture.
Knowledge and experience in the use of medical and mental health diagnoses, disabilities and treatment procedures. This includes acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology.
Knowledge of psychosocial treatment and ability to independently implement treatment modalities in working with individuals, families and groups who are experiencing a variety of psychiatric, medical and social problems to achieve treatment goals. This requires independent judgment and skill in utilizing supportive, problem solving or crisis intervention techniques.
Ability to independently provide counseling and/or psychotherapy services to individuals, groups and families. Social workers must practice within the bounds of their license or certification. For example, some states may require social workers providing psychotherapy to have a clinical level of licensure.
Ability to provide consultation services to other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment. Ability to provide orientation and coaching to new social workers and social work graduate students. Ability to serve as a field instructor for social work graduate students who are completing VHA field placements.
Ability to independently evaluate his/her own practice through participation in professional peer review case conferences, research studies, or other organized means.
Knowledge and skill in the use of computer software applications for drafting documents, data management, and tracking, especially those programs in use by VHA.
The full performance level of this vacancy is GS-11.
