This position is located in the Behavioral Health Department of the Fort Yuma Health Center (FYHC), within the Medical Services Division, in Winterhaven, CA. The incumbent is a member of the behavioral health interdisciplinary team and ensures patients receive quality centered care that is timely, appropriate, cost effective, sensitive and coordinated to affect desired clinical outcomes.This position reports to the Supervisory Social Worker onsite.
Summary

Independently provides expert counseling/therapy and follow-up services to patients (individual/family/group) to include supportive, family, group, cognitive, behavioral, interpersonal therapies. Assists in referral(s) to other resources and inpatient sites where indicated.
Provides case and program consultation for the development of comprehensive behavioral health services.
Assists in developing and assisting in gathering behavioral health data, and prepares factual information and report to assist in developing or evaluating new or existing programs in the area behavioral health.
Collaborates with and provides consultation to FYHC, schools, social services and judicial system as appropriate, Bureau of Indian Affairs, community services, law enforcement, etc., in promoting behavioral health and addiction prevention, treatment, aftercare, and follow-up program activities through individual, family, and group meetings
Provides academic trainings and may cover such issues as the disease concepts, treatments and follow-up process, prevention orientation, methods of prevention and treatment, youth and family issues, orientation to crisis prevention techniques, principles of patient confidentiality and other issues pertinent to behavioral within the Indian Community.

Qualifications

BASIC REQUIREMENT(S): Master's degree in social work from a school accredited by the Council on Social Work Education.In addition to the Basic Requirements, you must also meet the Minimum Qualifications stated below.
MINIMUM QUALIFICATION for the GS-12 Grade Level:
At least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Assisting patients and their families in utilizing available community, state, and federal resources for the purpose of improving their social, physical and psychological health; reporting on patient who are involuntarily committed; serving as liaison between patient, physician/clinic, or other members of the team including community, state, and federal agencies; completing monthly statistical and narrative reports on all services provided; provide individual and family therapy to out-patients
SELECTIVE PLACEMENT FACTOR:
In addition to the Basic and Minimum requirements, the following certification is determined to be essential to perform the duties and responsibilities of a particular position.
