To apply for this scholarship, you must provide a complete Application Package which includes:

4. Evaluations and Recommendations: VA Form 10-0941E
   1. Required: Academic Faculty: Preferably at your current school. If you just enrolled into the school and don't know anyone there, you can reach back to your previous level of education even if it is High School.
   2. Required: A person who has known you for at least 2-years. Preferably an employer/supervisor. If you were not employed, consider someone with strong credentials who can prepare a good recommendation for you.
   3. Required (VA Employees Only or who worked for the VA less than 3-years ago): Preferably the VA Supervisor or equivalent person if supervisor is no longer available.
   4. Only submit required recommendations. If an applicant submits recommendation that exceed the maximum of two for non-VA Employee and three for VA Employee, only the first two or three will be considered. All others will be discarded.
5. Résumé: The applicant's résumé supports information contained on all forms. Do not assume that the information on the résumé will automatically translate to information on the Application.
   1. Use the résumé to support all the information provided on the application and the answers provided on the Online Questionnaire.
   2. Include prior education, professional licenses, registrations, certifications, and detailed descriptions of volunteer and work experience, especially those that are healthcare related.
   3. The résumé must not exceed five single-sided pages and must be at least 11-point font.
   4. Dates should be in MM/YYYY format.
6. Transcript: NOTE: An unofficial transcript is acceptable from program currently in or previous transcript for courses completed.
   1. At a minimum, the student must provide an un-official transcript to support the CGPA the school put on the Academic Verification form.
   2. If there is not a supporting transcript for the CGPA, then the application will not be considered.
7. Declaration for Federal Employment: (OF-306) This form is required to complete the Background Investigation. If the applicant does not submit this form, the application will not be considered.
   1. Page 1 of the DFE should not be included in your application submission as it is an instructional page for the applicant.
   2. The information on the form is self-explanatory and all information should be filled in.
   3. Once you have filled in the information, print out the form and Sign/date line 17a.
8. Document Handling
   1. Because many of the documents should be printed and signed, you will need to re-scan those documents back into the computer so they can be prepared for final submission. Here are some notes when rescanning:
      1. The preferred file type is a .pdf.
      2. When scanning, set your dpi (dots per square inch) to 100. This will keep the file size down.

Veterans' Preference: When applying, eligible Veterans should claim preference for 5pt (TP), 10pt (CP/CPS/XP), or for Sole Survivor Preference (SSP) in the questionnaire. NOTE: Participation in the seasonal influenza program is a condition of employment and a requirement for all Department of Veterans Affairs Health Care Personnel (HCP). HCP are individuals who, during the influenza season, work in VHA locations where patients receive care or who come into contact with VA patients as part of their duties. VHA locations include, but are not limited to, VA hospitals and associated clinics, community living centers (CLCs), community-based outpatient clinics (CBOCs), domiciliary units, Vet centers and VA-leased facilities. HCP include all VA licensed and unlicensed, clinical and administrative, paid and unpaid, full- and part-time employees, intermittent employees, fee basis employees, VA contractors, students, researchers, volunteers, and trainees who work at these facilities. HCPs that are unable to receive or otherwise decline a flu shot will be expected to wear a face mask throughout the influenza season.
