Summary

The Social Worker is assigned to Substance Use Disorders Treatment Program (SUDTP) in the Mental Health Service Line, Southern Arizona VA Health Care System. The SW assigned to this position is either licensed to independently or under supervision, provide substance use disorders and mental health services to patients with substance use disorders and mental health needs. Services include diagnosis and treatment within the scope of knowledge, theory, and training of social work practice.

Duties

Help

Duties include, but not limited to:

  • Develops bio-psychosocial assessment of the patient's care needs in collaboration with the interdisciplinary treatment team, the Veteran, their family, and/or significant others.
  • Demonstrates an understanding of the range of treatment and skills needed for all young adult, middle adult, older adult, and geriatric patients related to their physical limitations, psychological needs and age associated illnesses and conditions.
  • Develops a recovery/treatment plan, and sets achievable recovery/treatment goals with the patient /family based on the bio- psychosocial assessment.
  • Selects and uses VA approved evidence based treatment methods appropriate to the needs of each patient.
  • Makes a rapid assessment of urgent patient care needs, provides crisis intervention and develops a crisis management plan including mobilizing community resources pertinent to crisis management and admission to inpatient mental health programs for patients in crisis who may present themselves to the center, call or are referred by other providers.
  • Implements recovery/treatment plans through the methods of care management, individual, group work, family therapy or other VA approved modes of interventions consistent with his/her scope of clinical proficiency.
  • Develops timely and viable discharge plans consistent with discharge needs.
  • Coordinates discharge plans with patient, treatment team and arranges necessary providers to meet discharge needs.
  • Establishes priorities for timely service delivery to prevent hardship for patient/family and prevent delays in access to and discharge from treatment.
  • Records all social work activity in medical records consistent with MHS policy.
  • Provides education related to VA and community resources, entitlements, Advance Directives/Living Wills and refers Veterans/family to the appropriate interdisciplinary team member for identified health education needs.
  • Collects and maintains up to date statistical data in spread sheet on patients treated in the SUDTP.
  • Describes duties all or in part may be provided in person and/or via tele-health.
Work Schedule: Monday - Friday, 8:00 am - 4:30 pm and may be required nights, weekends and holidays.

Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 03/02/2020.

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Basic Requirements:

  1. United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  2. Must be proficient in spoken and written English. To be appointed under authority of 38 U.S.C., Chapter 73 or 74, to serve in a direct patient-care capacity in Veterans Health Administration (VHA), applicants must be proficient in written and spoken English.
  3. Must have a Master's degree in Social Work (MSW) from a school of Social Work fully accredited by the Council on Social Work Education (CSWE).
  4. Must be licensed or certified by a state to independently practice Social Work at the Master's degree level. e.g. LMSW. (Required GS-11 Grade Level).
Grade Determinations: GS-09: New Masters of Social Work graduate, or otherwise qualified candidate, may apply pending completion of state prerequisites for licensure examination.

Grade Determinations: GS-11: Requires a minimum of one year of post-MSW experience equivalent to the GS-09 grade level in the field of health care or other social worker-related settings. This experience must have been in a clinical setting and must have demonstrated the potential to perform advanced assignments independently. (A clinical setting is a medical center, clinic - medical or psychiatric, residential treatment center, or any other type of facility where social work participates in a collaborative treatment and is identified with the medical profession.) Experience must have been obtained AFTER earning a Master's Degree in Social Work. OR SUBSTITUTING EDUCATION FOR EXPERIENCE AT THE GS-11 LEVEL: To meet the required experience for the GS-11 level, applicant may substitute a doctoral degree in social work from a school of social work. You must submit a transcript to receive credit for this education. and licensure or certification in a state at the independent practice level.

The full performance level of this vacancy is GS-11. The actual grade at which an applicant may be selected for this vacancy is GS-11.
