Duties

Help

The incumbent in this position functions as the Medical Center's Liaison for VA's Health Care for Homeless Veterans Programs, Homeless Providers Grant and Per Diem Program. The incumbent is responsible for monitoring and coordination of services provided by community-based programs funded under VA's Homeless Providers Grant and Per Diem Program.

The incumbent provides case management at Homeless Grant and Per Diem Program sites as described in the Homeless Grant and Per Diem program rules, regulations, and directives. The incumbent utilizes various treatment modalities of individual and group work, casework, community organization, and community outreach activities to alleviate or remove problems that interfere with the Homeless Grant and Per Diem Program participants' ability to achieve independence.

The incumbent monitors the clinical care and assesses the compliance of the programs receiving per diem as described in the grant recipient's original grant application. When necessary, the incumbent intervenes to facilitate compliance or corrective actions and, when appropriate, develops, facilitates and monitors plans of correction.

The incumbent provides services and oversight of the Homeless Grant and Per Diem funded programs as defined in the Grant and Per Diem rules, regulations, and directives including verification of Veteran status and eligibility for participation in the Homeless Grant and Per Diem Program, maintaining admission and discharge dates of participants for billing purposes, reviewing and approving/disapproving readmissions to and extensions of care, and collecting and submitting participant information as outlined by NEPEC and/or HOMES evaluations procedures.

The incumbent coordinates and participates in program site inspections utilizing the Grant and Per Diem Program rules, regulations, and directives as guidance and forwards completed inspection packages to the appropriate offices. When necessary, the incumbent notes inspection deficiencies and establishes methods to monitor corrective actions and develops timelines for consequences as·a result of inaction or insufficient actions related to deficiencies.

The incumbent coordinates care including serving as an advocate on behalf of the Veterans to ensure comprehensive service delivery, and linking and referring Veterans to the VA Medical Center, VA Regional Offices and/or community agencies.

The incumbent engages in coordination with community agencies, organizations, and groups to learn of services or resources available for homeless Veterans as well as provides current information on services and benefits available in the VA for Homeless Grant and Per Diem Program participants.

The incumbent complies with criminal conflict of interest laws and Executive Branch Standards of conduct so as to avoid conflicts of interest while serving as the Homeless Grant and Per Diem Program Liaison/Case Manager.

The incumbent participates in Medical Center research by identifying questions and trends that should be studied as a basis toward improving aspects of social service and/or physical or mental health care delivery to Veterans as well as devising and carrying out studies designed to test appropriate hypotheses.

The incumbent actively participates in the Grant and Per Diem Program conference calls and attends training offered by the Grant and Per Diem Program.

The incumbent actively participates in program planning and development, advising the supervisor of shifting trends with caseloads and identifying the need for and making recommendations on program procedure or policy changes based upon work experiences.


The incumbent participates in the orientation/teaching of graduate social work/nursing students, students of other disciplines, and medical center personnel on the bio-psychosocial aspects of homelessness.

The incumbent takes responsibility for improving professional's skills, participating in planning and executing specific staff development projects, attending seminars and institutes, and participating in training programs of other disciplines as requested.

The incumbent performs accountability functions: 1) document all contacts with patients/families in the medical charts; 2) completes statistical, administrative, staffing studies and all other reports in accordance with VA Medical Center and respective professional discipline service policies.

The incumbent works with VA Medical Center multi-disciplinary teams and community agencies to strive for developing more effective and efficient ways of delivering healthcare services to Homeless Grant and Per Diem Program Veterans.


Work Schedule: Monday - Friday 7:30 a.m. to 4:00 p.m. or 8:00 a.m. to 4:30 p.m.

Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 01/07/2020.

Basic Requirements:

Citizenship. United States Citizen

Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to the CSWE website to verify if that social work degree meets the accreditation standards for a master of social work.

Licensure. Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found on the OHRM website. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level and must be able to provide supervision for licensure.

GS-12 Social Worker Program Coordinators. Program coordinators at the GS-12 grade are administratively responsible for a clinical program providing treatment to patients in a major specialty, i.e., Suicide Prevention. They may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. They manage the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget. Individuals assigned as social worker program coordinator must be licensed at the advanced practice level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. They may have certification or other post-master's degree training for a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. In addition, the candidate must demonstrate the professional KSAs shown below.

Demonstrated Knowledge, Skills, and Abilities

  1. 1.Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program
  2. 2. Ability to supervise multidisciplinary staff assigned to the program.\
  3. 3. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
  4. 4. Ability to provide training, orientation, and guidance within clinical practice.
Preferred Experience: This is Community-base work, such as home visits or outreach. Experience with vulnerable/high-risk populations.

The full performance level of this vacancy is GS-12.

Physical Requirements: The physical demands of this position involve active listening, reading, observing and evaluating verbal and nonverbal behavior, walking, speaking, writing reports, writing policies and procedures, writing clinical notes, using office equipment such as computers, telephones, fax machines, photocopy machines, and driving throughout the VISN. A valid driver license is required. Strenuous physical activity is not required.
