VA Hudson Valley Health Care System is located in the heart of the Hudson Valley, New York, surrounded by amazing hiking, biking and walking trails as well as a host of wonderful outdoor activities including activities on the Hudson River. The Hudson Valley is a hotbed for culture - art, music, literature and more -- in towns, like Woodstock, Bethel, Rye, Sleepy Hollow, Tarrytown, Cold Spring and more. With historic locations like Hyde Park (home of President Franklin D. Roosevelt), Newburgh (the site of one of George Washington's Headquarters during the revolution) and the historic U.S. Military Academy, the area offers an abundance of single-person and family activities. In addition to the natural splendor of the area, VA Hudson Valley is also located 38 miles (a one-hour train ride) from New York City, home of the New York Yankees, Broadway shows, amazing museums and restaurants and so much more; a four-hour drive from Boston; and a 6-hour drive to Washington D.C. VA Hudson Valley 1,600 employees are committed to the more than 150,000 Veterans we serve in the region and look forward to the Hudson Valley as a backdrop for all we do each day.

PATIENT CENTERED CARE FOR THE VETERAN:

To deliver whole health, the Hudson Valley VA is applying the Elements of Patient Centered Care - a model that starts with an exploration of what matters most to Veterans and partners with them to achieve their best possible health.

Personalized: A personalized approach requires a dynamic customization of self-care and professional health care strategies. These must be
specifically relevant to the individual, based on factors such as their medical conditions, genome, needs, values, and circumstances.

Proactive: A proactive approach uses strategies that strengthen each patient's innate capacity for health and healing, such as mind-body therapies and nutritional changes to build resilience prior to surgery or chemotherapy.

Patient-driven: A patient-driven model is rooted in and guided by what matters most to the person, aligning their health care with their day-to-day goals and lifelong aspirations. This is the most critical component, because a personalized, proactive approach that is not based on the individual's values will have little impact. A person who is engaged and inspired is the most likely to achieve the highest possible level of health and well-being.

Education

There is no educational substitution at this grade level.
