Help

Serve as a Clinical Pharmacist Practitioner providing a full scope of healthcare services for patients with both acute and chronic illnesses and conditions covering the entire spectrum of medicine.
Assess, plan, treat, counsel, and provide guidance and instructions to patients with a variety of acute and chronic illnesses.
Provide follow-up and ongoing disease management for patients.
Examine and conduct a thorough evaluation of patients utilizing laboratory and clinical findings to appropriately treat or refer patients to the appropriate specialty clinic or ward.
Write prescriptions for legend and over the counter medications as approved by the Pharmacy and Therapeutic and Credentials Committees.

Appointment may be subject to a suitability or fitness determination, as determined by a completed background investigation.
Two year trial/probationary period may be required.
This position is subject to annual seasonal influenza vaccinations.
This position is a Testing Designated Position (TDP) subject to applicant testing and random drug testing.
A pre-placement medical examination is required.
Immunization screening is required. Hepatitis B immunization is required for all positions with direct patient contact. Applicants may be required to show proof of other immunizations depending on the type of position.
All Health Care Providers must be able to obtain and maintain current Basic Life Support (BLS) Training and certification that is sponsored or endorsed by the American Heart Association (AHA).
Applicants must be able to fulfill credentialing requirements and obtain and maintain appropriate/relevant clinical privileges.
This position requires shift work.
This is an Inclement Weather position.

Degree: Completion of a doctoral degree in Pharmacy that is recognized by the Accreditation Council for Pharmacy Education (external link) (ACPE) or an accrediting body recognized by the U.S. Department of Education (external link) at the time the degree was obtained.

License: Possess licensure to practice pharmacy in a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.

Medical Requirement: Applicants must be able to distinguish basic colors.
