The VISN 04 Physical Therapist (PT) Clinical Specialist (CS) acts as the subject matter expert in the specialty area of physical therapy to Physical Medicine and Rehab Departments (PM&R), medical center staff, and VISN 04 clinical staff virtually, and clinical rehabilitation veteran needs related to clinical resource hub (CRH) initiatives and implementation of VHA and the Joint Commission standards.
Summary
The VISN 04 Physical Therapist (PT) Clinical Specialist (CS) acts as the subject matter expert in the specialty area of physical therapy to Physical Medicine and Rehab Departments (PM&R), medical center staff, and VISN 04 clinical staff virtually, and clinical rehabilitation veteran needs related to clinical resource hub (CRH) initiatives and implementation of VHA and the Joint Commission standards.
The VISN 04 PT/CS provides point of service consultation and follow up care rehabilitation services by use of virtual care modality to all VISN 04 Medical Center, Community Based Outpatient Center (CBOC), and VISN 04 clinical staff. The incumbent is responsible for:
(A) Makes autonomous clinical decisions in a Direct Access environment.
(B) Interprets findings from examination and evaluation, select appropriate test and measures, and integrate findings.
(C) Determines physical therapy prognosis by incorporating examination findings with the patient's preferences
(D) Performs evaluations of individuals who have or may develop impairments, activity limitations, and participation restrictions related to conditions of the musculoskeletal, neurological cardiovascular, pulmonary, and/or integumentary systems
(E) Clinical skills in a range of specialized interventions and treatment modalities used in physical therapy.
(F) Guides the work of a multidisciplinary team.
(G) Develops and coordinate clinically productive relationships(
H) Teaches and mentors medical and VISN staff
(I) Serves as subject matter expert in Physical Therapy for VISN 04 staff.
(J) Develops and coordinates clinical education programs with evidence of advanced experience in clinical education.
(K) Consultant for health care providers regarding the delivery of care
(L) Knowledge of clinical signs and symptoms to make appropriate referrals
(M) Effectively performs verbal and written communication with patients, families, caregivers, and other healthcare professionals
(N) Provide in-service and clinical training programs
(O) Designs, conducts, and evaluates education, experiences within specialty content area for VISN 04staff members
(P) Establishes and maintains contact with other key staff involved in patient care(
Q) Develops and implements treatment plans designed to meet established specific, measurable, achievable, and time-bound (SMART) goals.
(R) Evaluates patient response to treatment and modifies the treatment plan
(S) Ensures accurate and timely defensive documentation(
T) Ensures appropriate and accurate selection and application of the correct reimbursement codes and workload entry as supported by documentation.(U) Recognize areas beyond the scope of PT practice and refer to most appropriate provider.
(V) Performs Physical Therapy intervention "on-demand".
(W) Supports and collaborates with staff of VISN 04 ,local facilities, and other stakeholders
(X) Conducts examinations, interventions, documentation, and follow-up evaluations at the VISN and VAmedical centers virtually and face to face, as indicated.
(Y) Works with veterans from a wide range of ages, backgrounds, and diagnoses.
(Z) Provides Physical Therapy representation at various clinical and administrative meetings/rounds at the discretion of the Supervisor.
(AA) Participates in multidisciplinary care by providing clinical guidance and expertise regarding goal planning and discharge needs related to PT for the CRH, VISN 04 and facility providers.
(BB) Provides clinical review and recommendations for continuation of community care PT to facilities within the VISN 04, needed.
(CC) Collaborates regularly with the Whole Health VISN 04 staff to promote the use of Whole Health services by performing Personal Health Inventories and other relevant assessments, developing Personal Health Plans and assisting veterans with choosing the most appropriate programs and clinical treatments.
(DD) Exercises independent problem solving capabilities.
(EE) Establishes and maintains contact with other key staff involved in veteran care via written, and verbal communication on regular basis.
(FF) Develops and provides timely reports to the Rehab and Extended Care Integrated Clinical Community(REC ICC) Subcommittee and Rehab Community of Practice (COP) describing relevant data demonstrating progress describing relevant data on implementation on PT virtually, & the status of expansion and sustainability across the network.
(GG) Develops program planning, Quality Improvement (QI) monitoring, and program evaluation within area of specialty on an annual basis.
(HH) Collaborates and supports medical centers' responses to formal, sensitive inquiries related to Veteran reported PT concerns as presented by Patient Advocacy, Executive Leadership, VISN 04, Congressional or others.
(II) Provides direct clinical care (including via telehealth modalities) at least 80% of official duty time with up to 20% allotted to care coordination, program development, and relevant administrative tasks. Assignments at the discretion of the supervisor.

Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. Individuals appointed under authority of 38 U.S.C. chapters 73 or 74, to serve in a direct patient-care capacity in VHA must be proficient in written and spoken English. See Chapter 2, section D, paragraph 5a.
LICENSURE:Individuals hold a full, current, and unrestricted license to practice physical therapy in a State, Territory or Commonwealth of the United States, or in the District of Columbia (must upload a copy)
Education and experience. The individual must meet at least one of the requirements below: (must upload a copy)
(1) Bachelor's degree in Physical Therapy AND five (5) years of progressively independent experience as a physical therapist.
(2) Master's degree in Physical Therapy AND two (2) years of progressively independent experience as a physical therapist.
(3) Doctorate degree in physical therapy
Grade Requirements: GS-13 Physical Therapist (Clinical Specialist): In addition to meeting the basic requirements, you must have one year of progressively complex experience equivalent to the next lower level GS-12 grade and one of the following:
Evidence of a minimum of 2,000 hours of clinical practice directly in the specialty area. This time must be between the past two and five years; OR
Board Certification in the specialty area recognized by the ABPTS; OR
Completion of a fellowship program; OR
An additional advanced degree in a related field.
Grade Determinations: In addition to to basic and grade requirements listed above you must also have one year of specialized experience equivalent to the next lower grade level GS 12. Examples of specialized experience are listed below and must be exhibited on your resume:
Monitors and makes work assignments, provides input on performance, resolves daily workplace issues and maintains efficient workflow.
Demonstrate the possession of advanced practice skills and judgment across many areas of physical therapy for both inpatient and outpatient clinics.
Provide clinical practice guidance for other staff PTs and assist with problem solving.
Determines treatment options for complex patients with multiple conditions.
Ensuring work is completed by distributing and balancing workload among employees in accordance with established work flow or job specialization; assuring timely accomplishment of the assigned workload; and assessing the status, quality and progress of work, and making day-to-day adjustments in accordance with established priorities.
Perform ancillary assignments, including program management duties on an occasional basis, where the complexity and amount of work is not substantial.
