Incumbent function as clinical specialist and provides chronic pain physical therapy services as an integrated part of the Pain Clinic and supports the Pain Management Team. Clinical Care will be provided through a combination of virtual and telehealth and in-person services. Incumbent will deliver collaborative and comprehensive pain care focused on teaching core non-pharmacological strategies to help patients better manage their chronic pain and improve their overall functioning and quality of life Incumbent who works at this level provides will also be able to participate in the Interdisciplinary Pain Rehabilitation Program(IPRP), Active Management of Pain (AMP) initiative which is supported by the Pain Management, Opioid Safety, Prescription Drug Monitoring Program (PMOP) with the Veteran's Health Administration (VHA) Specialty Care Program Office (SCPO).

This position is located in the Rehabilitation Department within Physical Medicine and Rehabilitation Service of VA Pittsburgh. The incumbent functions as a Physical Therapist with a primary duty station of the Monroeville CBOC, however they may be planned and spontaneous assignments across all rehab areas within the VAPHS catchment area. Clinical assignment may include outpatient, acute care, skilled nursing, Community Based Outpatient Care, in-home/in-community care, and virtual care.

Duties for this position include but are not limited to:

  1. Advanced knowledge and expert skill in a range of specialized interventions and treatment modalities used in a specialized treatment area of physical therapy.
  2. Ability to serve as a consultant and subject matter expert for health care providers regarding the delivery of care within a specialized content area of physical therapy.
  3. Ability to teach and mentor medical center staff in a specialized content area of physical therapy.
  4. Ability to guide the work of a multi-disciplinary team in a specialized content area of physical therapy.
  5. Advanced knowledge and expert skill to perform and interpret specialized procedures and tests in evaluating the treatment outcomes in the assigned specialty content area.
  6. Ability to make autonomous clinical decisions in a Direct Access environment through clinical privileges as a Licensed Independent Practitioner. This includes ability to independently evaluate and treat patients who seek physical therapy services. (Definition -Direct Access - As of January 1, 2015, all 50 states, the District of Columbia, and the US Virgin Islands allow patients to seek treatment from a licensed physical therapist without a prescription or referral. Patients are able to self-refer to physical therapists without being referred by a physician or other healthcare practitioner. Advanced clinical roles include managing direct access patients, as well as performing highly specialized clinical procedures such as dry needling, prescribing and training on assistive technology devices and durable medical equipment, ordering imaging studies and regenerative rehabilitation.
  7. Skill in performing examinations and evaluations of individuals who have or may develop impairments, activity limitations, and participation restrictions related to conditions of the musculoskeletal, neuromuscular, cardiovascular, pulmonary, and/or integumentary systems while considering the effects attributable to unique psychosocial and environmental factors. In cases with complex medical needs, the therapist is expected to consult with individuals with more experience and knowledge for direction in treatment.
  8. Ability to interpret findings from examination and evaluation, select appropriate test and measures, and integrate findings into the physical therapy plan of care for the full range of patient populations, with focused training and skill set applicable to chronic pain diagnosis and treatment.
  9. Ability to establish a diagnosis within the scope of physical therapy and identify the appropriate rehabilitation intervention, including referral to another provider for further consultation as clinically indicated.
  10. Ability to determine physical therapy prognosis by incorporating examination findings with the patient's preferences in order to set clinically appropriate treatment goals, optimize outcomes and maximize functional independence.

Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy
  • Education and experience: You must meet at least one of the requirements below NOTE: Individuals must be a graduate of a Commission on Accreditation in Physical Therapy Education (CAPTE) accredited college or university.(1) Bachelor's degree in Physical Therapy AND five (5) years of progressively independent experience as a physical therapist.(2) Master's degree in Physical Therapy AND two (2) years of progressively independent experience as a physical therapist.(3) Doctorate degree in physical therapy.
  • Licensure: Individuals must hold a full, current, and unrestricted license to practice physical therapy in a State, Territory or Commonwealth of the United States, or in the District of Columbia.
  • English Language Proficiency: Individuals appointed under authority of 38 U.S.C. chapters 73 or 74, to serve in a direct patient-care capacity in VHA must be proficient in written and spoken English. See Chapter 2, section D, paragraph 5a, this part.May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations:

GS 13 Physical Therapist (Clinical Specialist)


(a) Experience. In addition to meeting the basic requirements, one year of progressively complex experience equivalent to the GS-12 grade level and one of the following:
1. Evidence of a minimum of 2,000 hours of clinical practice directly in the specialty area. This time must be between the past two and five years; or
2. Board Certification in the specialty area recognized by the ABPTS; or
3. Completion of a fellowship program; or
4. An additional advanced degree in a related field. In addition, the candidate must demonstrate the all following KSAs.

(b) Demonstrated Knowledge, Skills, and Abilities
1. Advanced knowledge and expert skill in a range of specialized interventions and treatment modalities used in a specialized treatment area of physical therapy.

Receiving Service Credit or Earning Annual (Vacation) Leave: Federal Employees earn annual leave at a rate (4, 6 or 8 hours per pay period) which is based on the number of years they have served as a Federal employee.
