This pharmacist promotes and manages drug therapy based on current clinical knowledge that is consistent with policies established at the national, VISN, and local levels. The hematology/Oncology Clinical Pharmacy Practitioner functions as part of the Medical Team and is most effective when assigned to a team, or panel of patients, to support continuity of care. Clinical Pharmacy Practitioner (CPP) are employees of the medical center's Pharmacy Service, to ensure they receive all professional guidance, policy and communications, along with department missions and goals, especially regarding the VA National Formulary, VISN and local medical center Pharmacy and Therapeutic Committee objectives for formulary management activities and patient safety. If the CPP if an employee of a different medical center service, the CPP scopes will be maintained by Pharmacy Service. The clinical pharmacy practitioner is able to perform all the clinical and dispensing functions listed in the clinical pharmacist functional statement however spends the majority of time working at the top of their license.

Functions, Duties:

1. Is directly involved in the design, implementation and monitoring of therapeutic drug plans to achieve definite outcomes through direct interactions with patients and providers in assigned areas.
2. Obtains medication histories from patients and summarizes significant findings in the medical record and/or to the provider as appropriate.
3. Is actively involved in reviewing patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes as required. Communicates findings with prescribers and provides appropriate alternatives to current treatment plans as needed.
4. Monitors for and reports drug errors, adverse drug reactions, allergies, and patient compliance issues. Documents findings per facility procedures.
5. Is able to apply knowledge of normal laboratory values in the evaluation of patient care and recognizes significant abnormalities.
6. Has expertise in the principles of clinical pharmacokinetics and pharmacodynamics and is knowledgeable and able to make dose adjustment recommendations based on objective laboratory findings.
7. Reviews and evaluates requests for non-formulary and restricted drugs for appropriateness and compliance with established criteria where applicable.
8. Assists with medication use evaluations and other Pharmacy and Therapeutics activities.
9. Documents clinical interventions in CPRS in a timely and professional manner as appropriate.

Drug Information:

1. Serves as an authority on drug usage, interactions, over-dosages, and compliance to medical staff, nursing staff, technicians and students. Uses appropriate references to research drug information.
2. Has a good working knowledge of information resources, both written and electronic.
3. Maintains a current knowledge of therapeutics and disease management.
4. Provides timely and accurate responses to drug information inquiries from all customers.

1. Automated Data Processing (ADP) Security - In the performance of official duties, the employee has regular access to printed and electronic files containing sensitive information, which must be protected under the provisions of the Privacy Act of 197 4, Health Insurance Portability and Accountability Act (HIPAA) of 1996, and other applicable laws, and regulations. The employee is responsible for protecting that data from unauthorized release or from loss, alteration or unauthorized deletion and adhering to the provisions of the ADP security agreement signed by the employee.
2. Maintains patient confidentiality.
3. Adheres to all medical center and department policies and regulations.

Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Education (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.) (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
  • Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.
  • English language proficiency in both written and spoken English.
  • Physical requirement: pre-employment physical required.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations GS-12:
  • In addition to the basic requirements, candidates must be able to show completion of an ACPE-accredited Pharm.D. program, OR 1 year of specialized experience equivalent to the next lower grade level.
Specialized Experience GS-12: In addition to the basic requirements, candidates must demonstrate the following knowledge, skills and abilities (KSAs):
  1. Knowledge of professional pharmacy practice.
  2. Ability to communicate orally and in writing to both patients and health care staff.
  3. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.
  4. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level

Clinical Pharmacy Specialist: The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. Pharmacists assigned to this position must demonstrate the following KSAs:
  1. Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
  2. Expert understanding of regulatory and quality standards for their program area.
  3. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
  4. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
  5. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

Preferred Experience:
Doctor of Pharmacy degree. Preference will be given to those with a), b) or c) below:


a). Three (3) years of practice experience with a substantial component (>50%) of patient care activities in pharmacotherapy.
b). Completion of a PGY1 and/or PGY2 Hematology/Oncology Residency or equivalent post-graduate training with a substantial component (>50%) of patient care activities in pharmacotherapy
c). Board certification recognized by the American Pharmaceutical Association's Board of Pharmaceutical Specialties, or the American Society of Consultant Pharmacists (or equivalent). (Board Certified Oncology Pharmacist)
