The Cincinnati, OH VA Medical Center is looking for a well-qualified Physician to fill a full time Physician position located within the Primary Care Department. This Primary Care Physician will serve as a Float / Gap Provider.
This PACT Physician will report directly to the Associate Chief of Staff for Primary Care (ACOS/PC).
Work Schedule: Monday-Friday, 8 a.m. - 4:30 p.m.
Summary

This Primary Care Staff Physician will function as float or gap provider. This physician will fill in when there is a vacant PACT panel or if a new PACT position is created, or when a PACT Primary Care Provider (PCP) is on leave, etc.
Medical care is delivered in accordance with current clinical practice guidelines with attention to prevention via utilization of evidence-based clinical decision-making tools. In addition to the provision of high-quality medical care, PACT Physicians demonstrate excellence in customer service via compassionate and informed interpersonal relationships with Veterans, family members and staff.
Duties and responsibilities include, but are not limited to the following:
Functioning at the full extent of the team member's relevant clinical privileges, credentials, scopes of practice, elements of practice, certification, functional statement, or other VHA or local facility approved documentation of competency.
Functions as the Physician Team leader of a multidisciplinary Patient Aligned Care Team (PACT).
Provides comprehensive, collaborative acute and continuity care to an assigned panel of Veterans as well as to patients outside of the assigned panel when Veteran care indicates a need for such.
Demonstrates flexibility in responding to clinic demands, adhere to personal and professional obligations and responsibilities, practice and maintain ethical standards, and sustain a work environment that is harmonious and customer service oriented.
Provide ongoing medical care to patients with acute and chronic health problems.
Refer to specialists when additional input is desired and required for symptom and disease management.
Provide preventative medical care including screening and vaccinations.
Participating in team performance improvement and sustainment activities to optimize team efficiency and care delivery to patients.
Managing communications and facilitating safe transitions of patients between the PACT's site of care and other health care settings, using informal and formal communication methods, as appropriate.
Possess excellent communication skills and relations with PACT and hospital staff and be able to elicit cooperation from others. This includes interacting in person, via email, phone, chart, and other forms of communication.
Respond to patient inquiries that arise from daily practice, through the Patient Advocate, through the Front Office, and by Congressional action.
Ensuring the patient has same-day access for face-to-face and telephone care visits during regular clinic hours.
Collaborating with PACT staff to develop personal health plans that incorporate care management and care coordination appropriate to the patient's needs.
Communicating with facility leadership regarding the resources needed by the PACT for optimal function.
In this position you will travel to other sites within the Health Care System to cover provider shortage. Coverage scenarios and locations may change on a daily basis based on patient care needs. The ability to be present at any of the Cincinnati, OH VA Medical Center (CVAMC) locations is essential. If for any reason a government vehicle is unavailable for use, the Float provider is still responsible for arriving at their assigned location on time. Private vehicle usage will be reimbursed based on existing travel pay criteria. Failure to maintain the ability to be present at any of the CVAMC location may lead to disciplinary action.

Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed.
Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.
Residency Training: Physicians must have completed residency training, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification. (NOTE: VA physicians involved in academic training programs may be required to be board certified for faculty status.) Approved residencies are: (1) Those approved by the Accreditation Council for Graduate Medical Education (ACGME), b) OR (2) Those approved by the American Osteopathic Association (AOA),OR (3) Other residencies (non-US residency training programs followed by a minimum of five years of verified practice in the United States), which the local Medical Staff Executive Committee deems to have provided the applicant with appropriate professional training and believes has exposed the physician to an appropriate range of patient care experiences.
Residents currently enrolled in ACGME/AOA accredited residency training programs and who would otherwise meet the basic requirements for appointment are eligible to be appointed as "Physician Resident Providers" (PRPs). PRPs must be fully licensed physicians (i.e., not a training license) and may only be appointed on an intermittent or fee-basis. PRPs are not considered independent practitioners and will not be privileged; rather, they are to have a "scope of practice" that allows them to perform certain restricted duties under supervision. Additionally, surgery residents in gap years may also be appointed as PRPs.
Proficiency in spoken and written English.
Driver's Licensure - Must possess a active, valid driver's license.
Preferred Qualifications:
Complete 3-year residency in Family Medicine or Internal Medicine
Board Certified in Family Medicine or Internal Medicine.
Unrestricted DEA license.
Experience in geriatrics and longitudinal patient care is desired

Physical Requirements: Per VA Directive and Handbook 5019 - a physical examination will be conducted based on the physical requirements of the position. A physical examination prior to placement is required. The physician should be physically and mentally capable to perform the duties of the position including examinations, formulate the diagnosis and treatment plan and educate residents. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. Physical activities include potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing; heavy lifting (45 pounds or over), reaching above the shoulder, repeated bending, use of fingers; use of both hands required; walking (8 hours); standing (8 hours); ability for rapid mental and muscular coordination simultaneously; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in one eye to 20/20 and to 20/40 in the other; ability to distinguish basic colors; ability to distinguish shades of colors; hearing (aid permitted); mental and emotional stability; and must have clear speech. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to wear protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Rigid deadlines and need for high degree of accuracy periodically becomes stressful. Must be able to pass a driver's physical.
Note: Must have Basic Life Support (BLS) certification prior to assuming clinical duties. The BLS requirement is satisfied by successfully completing an American Heart Association (AHA) BLS Healthcare Provider Course or Military Training Network (MTN). BLS courses are normally offered within 1-5 days of entry on duty. No online certifications will be accepted.
Education
Degree of Doctor of Medicine or an equivalent degree resulting from a course of education in allopathic medicine or osteopathic medicine.
The degree must have been obtained from an institution whose accreditation was in place for the year in which the course of study was completed.
Approved schools are:
Schools of medicine accredited by the Liaison Committee on Medical Education (LCME) for the year in which the degree was granted, or
Schools of osteopathic medicine approved by the Commission on Osteopathic College Accreditation of the American Osteopathic Association for the year in which the degree was granted.
For foreign medical graduates not covered in (1) or (2) above, facility officials must verify with the Educational Commission for Foreign Medical Graduates (ECFMG) that the applicant has met requirements for certification, and must obtain a copy of the ECFMG certificate, if claimed by the applicant. [If the applicant does not claim an ECFMG certificate, facility officials must still confirm that the medical school meets (or met) ECFMG eligibility requirements for the year the candidate graduated.]
NOTE: The Under Secretary of Health or designee in the VHA Central Office may approve the appointment under authority of 38 U.S.C. § 7405 of a physician graduate of a school of medicine not covered above if the candidate is to be assigned to a research, academic, or administrative position with no patient care responsibilities.
