This Supervisory Social Worker position is located at the Mattoon CBOC and has clinical responsibility for PACT, provides Mental Health coverage when needed, and supervises medical Social Workers in the Community Based Outpatient Clinics. The Supervisory Social Worker independently provides psychosocial assessment, therapy, and case management services related to PACT/Mental Health needs at an advanced practice level to eligible veterans and their family members. Services provided relate to psychosocial implications of psychiatric disorders, homelessness, substance abuse, post-deployment issues, medical case management, psychosocial treatment, post hospital care and rehabilitation. The CBOC's cover a wide catchment area; often with few resources. This requires a high degree of clinical skill and creative problem solving. The Supervisory Social Worker must work closely with community agencies and CBOC staff to meet all the needs of the Veterans and their families.

The CBOC Supervisory Social Worker performs a wide range of clinical and administrative functions serving the CBOC Veterans. These functions include:

  • Assesses and treats complicated psychosocial problems.
  • Assists Primary Care Provider in setting up needed services to address complex medical needs, i.e., home IV therapy, hospice, or wound care.
  • Addresses home care needs, homelessness, and transition across levels and sites of care.
  • Helping Veterans and their families maximize treatment potential and achieve more satisfying and productive emotional and social functioning.
  • Provides social work psychosocial assessment, diagnosis, and treatment in collaboration with interdisciplinary team members to develop a care management plan and psychosocial interventions.
  • Performs specialized treatment for complex mental health needs and is able to assess, utilize differential diagnosis and provide short-term treatment modalities of individual, group, marital and family therapy services.
  • Completes the initial MH triage for all Veterans referred for mental health or substance abuse services at the CBOC.
  • Develops crisis management plans for in-patient psychiatric admissions or appropriate safety plans for deflections.
  • Provides case management and supportive services to families as indicated.
  • Must be sensitive to the ethnic and cultural diversity and age-specific challenges of the Veteran population served and adjusts intervention and treatment plans as appropriate.
  • Develops and uses appropriate community resources.
  • Serves as an advocate for Veterans and their families, helping them access needed services at the facility, at other VA facilities, and in their local communities.
  • Assists Veterans and their families with advance directives, guardianships, and applications for home care, nursing homes, adult daycare, and extended care services.
  • Participates in the orientation, training, and teaching of social work graduate students and other trainees and staff.
  • Provides in-services to other CBOC staff in the areas of mental health and improving care management procedures.
  • Prepares monthly and quarterly statistical reports as required for care management and OEF/OIF screenings and monitors.
  • Conducts and/or participates in research and/or program evaluation as appropriate.
  • May serve on committees, work groups, and task forces at the facility, VISN, and/or Central Office levels.
  • Responsible for scheduling appointments and follows all guidelines and policies regarding scheduling, and entries into EWL.

Qualifications

To qualify for this position, applicants must meet all requirements within 30 days of the closing date of this announcement.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency: Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j, this part.
Education: Must have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. (TRANSCRIPTS REQUIRED)
Licensure: Must be licensed or certified by a state at the advanced practice level and must be able to provide supervision for licensure. (COPY OF LICENSE REQUIRED)

GS-12 Grade Determination: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills in a specialty area or in administration demonstrating progressively more professional competency and judgment.

In addition to the experience above, the candidate must demonstrate all of the following KSAs:

  • Ability to independently organize work, set priorities, and meet multiple deadlines.
  • Knowledge of supervisory duties which includes clinical supervision, consultation, negotiation with other departments and quality improvement.
  • Ability to ensure provision of clinical social work services.
  • Ability to provide staff training and development.
  • Skill in interpersonal relationships in dealing with employees, team leaders, and managers.
