The overall purpose of this position is to help patients maintain, regain or enhance their level of social functioning when their ability to function is affected by actual or potential illness, disability or injury. The contents of these services range from prevention and education to helping patients deal with major changes in social roles, future expectations and life goals precipitated by changes in health status. - Assesses and evaluates patients using a patient/family-centered approach for needs such as, discharge planning/coordination, long-term care placement, and other psychological concerns related to health care.
- Develops intervention based on above need assessment and exercises mature professional judgment in consultation with the supervisory social worker in providing problem-focused, culturally appropriate care.
- Participates and when appropriate takes a leadership role in assessing community human service needs. Works with community members and human resources providers to develop approaches to addressing the needs.
