Physician Cardiologist - Heart Failure

  • Provide comprehensive, guideline directed, evidence based care to patients with heart failure
  • Implement assessment, diagnosis and management of chronic heart failure patients with complex co-morbidities
  • Obtain health histories, perform physical exams and documents findings in the medical record
  • Order appropriate diagnostic procedures and act on the results
  • Prescribe individualized pharmacological and non-pharmacological interventions for treatment of heart failure and related medical problems for the promotion of quality of life, wellness, and improved outcomes as demonstrated by a reduction of hospitalizations or urgent care visits
  • Educate the patient, significant others and affiliated medical center staff on health care issues pertinent to the management of heart failure (i.e. medications, diet, lifestyle changes and follow up)
  • Facilitate the completion of informed consents prior to a procedure
  • Provide cardiology focused in-services and education programs to the medical center staff and affiliates
  • Participate in performance improvement initiatives
  • Demonstrate flexibility by providing care and coordination of care for patients with heart failure wherever it is needed, including the inpatient, emergency department, and outpatient setting
  • Collaborate with multiple other disciplines including primary care, hospice and palliative care
  • Manage patients with heart failure in the outpatient, ED, or inpatient setting
  • Optimize medications to improve heart function
  • Monitor and manage fluid volume changes
  • Refer patients to Care Coordination Home Telehealth (CCHT) as needed
  • Communicate with the patient's primary care and/or specialty care provider to provide an update on their clinical condition and management status
  • Participate in quality improvement projects
  • Identify and intensively manage patients who are at a high risk of readmission
Work Schedule:
  • Monday - Friday, 8:00 am - 4:30 pm
  • Part time (30 hours per two week pay period)

Qualifications

Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed.
  • Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.
  • Completion of residency training, or its equivalent, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification.
  • Proficiency in spoken and written English.

Preferred Experience:
  • Diverse general Cardiology experience
  • Heart Failure Management Training and Experience
  • Certified or board eligible in Cardiology
  • Advanced training in heart failure management
  • Currently in an ACGME accredited advanced heart failure and transplant cardiology fellowship program

Physical Requirements: Walking, bending, stooping, and some lifting in the direct care of patients. There may also be extended periods of sitting.
