Summary

Conducts Medical Staff authorized outpatient pharmacy clinics for examination, Follow-up and treatment of specialized patients. Takes the appropriate health history, follows and adjusts the developed treatment plan and monitors the patient's outcome to the prescribed treatment plan.
Evaluates medication regimens for appropriateness, drug interactions, therapeutic duplications, non-compliance and adverse drug events.
Counsels patients on the use, storage, cautions and side effects of all medications dispensed as well as documentation of cognitive services provided.
Provides highly complex, critical medication treatment, monitoring, management and consultation involving risk to patient's life, safety, health or well- being with all levels of allied health providers and patients.
Provides formal and informal education and in-services to medical staff, pharmacy, and nursing staff as requested. Provides training for pharmacy residents, pharmacy students and medical students.Provides education to patients and their families about medications, common disease states and information on health promotion.

May work on a regularly assigned, rotation or call back basis to provide continuity of services during evenings, nights, holidays, and weekends.

Qualifications

Experience:Applicant's qualifications background must demonstrate the knowledge, skills, abilities, and competencies necessary to perform the work of the position. Pharmacy work requires knowledge of the use, clinical effects, and composition of medications, including their chemical, biological, and physical properties. Qualifying professional pharmacy experience may involve, but is not limited to:
Dispensing medications prescribed by physicians and other health practitioners and providing information to health practitioners and patients about proper usage of medications and side effects;
Evaluating medication use patterns and outcomes for patients in hospitals or managed care organizations;
Performing administrative, consultative, or staff advisory work for a medical facility's pharmacy program;
Planning, monitoring and evaluating medication programs or regimens;
Establishing medication-handling procedures for the storage and preservation of medications;
Researching medical literature and/or clinical medication information to provide accurate responses to inquiries; and/or
Maintaining all medication records required by law.
In addition, to the Basic Requirements, you must also meet the Minimum Qualifications stated below-
MINIMUM QUALIFICATIONS:
GS-11: candidates must have3 years of progressively higher level graduate education leading to a Ph.D. degree, Ph.D., Pharm.D. or equivalent doctoral degree. GS-12: In addition to the requirements for the GS-11 (or equivalent) level, applicants must have a minimum of one year of professional pharmacy experience. Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: performing duties such as providing inpatient pharmacy services and the preparation of the complete range of sterile products, parenteral, enteral, and parenteral nutrition, and intravenous additives for a large medical facility; taking steps to prevent drug reactions or interactions; ensuring correct labeling, handling and storage of pharmaceutical products; advising physicians of possible safety issues; compounding drugs as prescribed, including IV and chemotherapy drugs; teaching pharmacy techniques/procedures to other pharmacists; or experience conducting studies of drugs to determine their effectiveness and safety.
The selected individual is required to obtain and maintain any licensure requirements. If licensure are not obtained or maintained during employment, the employee will be subject to adverse actions, up to and including removal from Federal service.
