Participates in drug monitoring therapy through direct patient care involvement, in consultation with primary physician, provides expertise in drug therapy, proper drug utilization, dosages, routes and methods of administration, adverse effects and interactions and assist in the evaluation of individual patient drug therapy. Inpatient/Outpatient orders are checked to include patient information, correct medication dose, route and frequency of administration. Orders are checked for patient's allergies, drug interactions, incompatibilities, duplication and priority. Identifies, interprets and resolves prescribing and dispensing issues, including pharmacy therapeutics and the formulary. Dispenses controlled substances with accuracy according to current policies and procedures for inpatients and outpatients.
Work Schedule: Incumbent will be assigned to work rotating shifts including Weekends and Holidays. Required to work 2nd Shift-From 3:00 pm to 11:30 pm or 3rd Shift-From 11:00 pm to 7:00 am. Required to be on Call. Main Shift: 2nd & 3rd Shift.
