The incumbent provides care management in the ambulatory health care setting.
Summary

Collaborate with primary care providers, clinic nurses, public health nursing, DM case management and social services to ensure that the patient's care needs are completed in a timely and cost effective manner to maximize each visit.
Serves as primary contact/clinical liaison between referral hospitals, treatment facilities, and patients and their families to ensure delivery of effective and appropriate treatment in accordance with established plan of care.
Contacts patients for follow-up diagnostic tests, referrals, appointments, or other medical issues as appropriate in collaboration with the provider.
Interacts with the patient/family/caregiver and primary care provider to develop and discuss the patient's plan of care, understanding of disease processes, and coordination of services based on patient's clinical needs and available resources.
Participates in departmental orientation, on the job training, and quality improvement programs/initiatives.
Coordinates clinic visits for assigned patients with other members of the multidisciplinary team in order to optimize care.
Conducts intake assessment, screening, and follow-up interviews with client and/or family members and/or partner(s) at office, by telephone, in the clinic, or at the home of the client.

A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required.
