Help

The Clinical Pharmacy Specialist (Ambulatory Care) is a licensed pharmacist with a Doctor of Pharmacy degree and completed an American Society of Health Systems Pharmacists accredited Clinical Pharmacy Practice Residency, or other equivalent experience. The incumbent will work with the Interdisciplinary PACT and specialty team(s) to assist in comprehensive medication management, therapeutic monitoring, lab ordering and monitoring, and patient education. The incumbent performs the following duties as assigned, but is not limited to:

  • The incumbent will provide education to staff in the ambulatory care setting.
  • The CPS may be assigned clinical cross-coverage duties as needed to meet the needs of the medical center.
  • Assists in other patient care areas as deemed necessary to maintain the consistency of overall pharmacy services.
  • All inpatient/outpatient orders are checked to include patient information, correct medication, dose, route, and frequency of administration. Orders are checked for patient allergies, drug interactions, incompatibilities, duplication and priority.
  • Identifies, interprets, and resolves prescribing and dispensing issues, including pharmacotherapeutics and formulary. Documents interventions as necessary.
  • Acts as a provider, with specialty training in Ambulatory Care with a scope of practice, providing expertise to other providers, pharmacists, nursing and other disciplines, as appropriate.
  • Provides oral and written consultation with other health care professionals regarding drug information, drug therapy selection observations on patient response to therapy, and appropriate recommendations regarding treatment alternatives or additional interventions to maximize patient outcomes taking into consideration choice of therapy, safety, efficacy, formulary, pharmacoeconomics, adverse drug reactions, allergies and disease states. Information and/or changes is communicated to the interdisciplinary team in an accurate, timely, and professional manner.
  • Designs and/or participates in research studies (including drug utilization evaluations) with providers and/or other research collaborators.
  • Determines staff development needs and assists in development of formal/informal programs to enhance pharmacist and technician understanding relating to ambulatory care Clinical Pharmacy Practice.
  • Works to develop, upgrade, and disseminate acceptable criteria for use for use of formulary and non-formulary medications and other high cost items directed at employing cost effectiveness without compromising the quality of patient care. Dissemination of information may occur in a setting requiring speaking in front of a large group of providers.
  • Participates in self-development activities to maintain and increase professional competence.
  • Effectively communicates all appropriate information and significant events through proper channels, when appropriate.
  • Other duties as assigned.
Work Schedule: Monday - Friday, 07:30 am to 4:00 pm; per the needs of the service.

Basic Requirements

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).
  • Education: (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy. Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
  • Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure.
Grade Determinations

The full performance level of this vacancy is GS-12. The actual grade at which an applicant may be selected for this vacancy is in the range of GS-12 to GS-13
  • Experience: Must have 1 year of specialize experience equivalent to the next lower grade level (GS-12).
  • Assignments: Candidates at this grade level are to be in one of the assignments listed below. For all assignments above the full performance level, the higher level duties must consist of significant scope, administrative independence, complexity (difficulty) and range of variety as described in this standard at the specified grade level and be performed by the incumbent at least 25% of the time. The Clinical Pharmacy Specialist (Ambulatory Care) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy.
Pharmacists assigned to this position must demonstrate the following knowledge, skills and abilities (KSAs):
  • Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
  • Expert understanding of regulatory and quality standards for their program area.
  • Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
  • Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
  • Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
Preferred Experience: PGY-1 Pharmacy Residency required, PGY-2 Pharmacy Residency in Ambulatory Care, Board Certified in Pharmacotherapy or Ambulatory Care, and/or Ambulatory Care experience.

Physical Requirements: Applicants must be able to perform the following duties: lifting (up to 50 lbs.); carrying (up to 50 pounds). Applicants must have use of all fingers. Walking or standing for up to 8-hours. Kneeling intermittently multiple times for up to 8-hours. Use of both legs is required. Must possess visual acuity to read typed print. Keen hearing (aids permitted). Mental stability must be required to handle rigors of the position. The loss or impairment of a specific function may be compensated by satisfactory use of prosthesis.
