Summary

Help

  • Routinely identifies the customer's situation properly and performs the tasks required to resolve the customer's problem appropriately and in a timely manner, including both internal and external customers
  • Assures Resident's dignity is maintained, such as coverage of drainage bags, clean/shaven, nails clean and trimmed, incontinent pads are not visible on beds, Residents are clean and neat in appearance always, privacy during provision of care
  • Assures that Restorative Care measures are implemented for each Resident assigned as identified in the Resident's plan of care
  • Supports the Resident in successfully coping with his/her physical limitations, environmental barriers and social prejudices that he/she encounters
  • Actively participates in the process of retraining the Resident to return to the community through creative and individualized approaches that restore the Resident's feeling of wholeness and self-respect
  • Implements personalized approaches to care for Residents with Dementia that address behavioral expressions of unmet needs utilizing non-pharmacological interventions
  • Incorporates approaches to his/he care that promote optimal physical functioning, nutrition and hydration needs and enhances the quality of life; also, incorporates environmental interventions that minimize distress
  • Accurately observes and in a timely manner reports to the Charge nurse and/or the IDTT team member's adjustments that may be indicated in the rehabilitation interventions to improve the Residents physical and functional status to successfully achieve goals within designated timeframes whenever possible
  • Consistently responds to requests or assistance, from patients, families, visitors and co-workers, promptly and in a friendly, respectful and cooperative manner
  • Supports the customer service mission of the VHA facility by dressing professionally and exhibiting a professional demeanor and attitude in all customer interactions, including face-to-face, telephonic, and e-mail interactions
  • Meets customer/patient expectations, with results to be determined through an established customer survey or other appropriate feedback system and has baseline measurements of customer/patient satisfaction
  • Consistently provides care in a trustworthy, compassionate, and respectful manner that honors Veteran preferences in regard to bathing, wake/sleep cycle, how a Veteran prefers to be dressed and groomed, oral hygiene practices, and diet choices
  • Implements cultural transformation activities and encourages a home like environment including encouraging Veterans to bring items from home and assist them to arrange their environment as they prefer
  • Encourages activities for Veterans throughout the CLC, including nursing led unit-based activities
  • In accordance with Nursing Service Procedure Manual, performs basic technical and rehabilitative nursing procedures for several patients with complex needs
  • In accordance with Nursing Service procedure, documents nursing procedures performed and their effectiveness accurately and in a timely manner
  • Assists with care appropriate to the patient's stage of development and incorporates patient's cultural values and background into the care delivered
  • Consistently recognizes a patient's unusual response to a nursing procedure and reports such response to the charge nurse in a timely manner
  • Initiates emergency procedures such as Code Blue, CPR, Heimlich maneuver when indicated
  • Accurately observes patients' vital signs, skin condition, intake, elimination, affect, behavior, and motor activity
  • Documents pertinent observations accurately and in a timely manner
  • Recognizes unusual observations and reports these observations to the charge nurse in a timely manner
  • Provides verbal and/or written information that is thorough and correct, using clear and precise communication methods to ensure that customers understand
  • Communicates with customers in a courteous, tactful, and helpful manner
  • Adheres to the Medical Center policies on confidential matters and patient confidentiality
  • Possesses knowledge of basic functions of VISTA/CPRS and Outlook, and maintains security in the use of the computer and the medical record.

Qualifications


Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Education: Must have one year above high school with courses related to the NA occupation. Completion of an intensive, specialized, occupation-related course of study of less than one year as a nursing assistant may also meet in full the experience requirements for GS-3.
  • English Language Proficiency: Must be proficient in spoken and written English in accordance with VA Handbook 5005, part II, chapter 3, section A, paragraph 3j.

Grade Determinations:

GS-5 Nursing Assistant
One year of progressively responsible assignments and experience equivalent to the GS-4 level which demonstrates knowledge, skills, and abilities that are directly related to the specific assignment. In addition, the candidate must demonstrate the professional KSAs in subparagraph (c) below;

OR,

Successful completion of a 4-year course of study above high school leading to a bachelor's degree that included 24 semester hours of courses related to health care or possession of a bachelor's degree

Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs:

(a) Ability to assist in the full range of nursing care to patients/residents with physical and/or behavioral problems in a hospital, long term care or outpatient setting under the direction of a Registered Nurse and/or Licensed Vocational Nurse/Licensed Practical Nurse.
(b) Ability to communicate orally with patients/residents, families, interdisciplinary team and other personnel. This includes serving as a preceptor to new [NAs] by assisting with the coordination of their orientation and overseeing/assessing their practical experience while in a clinical setting
(c) Ability to recognize and react to emergent patient/resident care situations and intervene while waiting for assistance. For example, recognizing need for basic life support, controlling bleeding and assisting with behavior crisis, etc.

The full performance level of this vacancy is GS-5.

Receiving Service Credit or Earning Annual (Vacation) Leave: Federal Employees earn annual leave at a rate (4, 6 or 8 hours per pay period) which is based on the number of years they have served as a Federal employee.
