Duties include but are not limited to the following:
Demonstrates ability to collaborate with all levels of services and disciplines. Collaborates with patient, family and care providers in any and all settings where care is being provided to evaluate and update changes inthe patient-driven life plan and patient management. Coordinates and manages patient care included on the high risk registry by utilizing the Care Assessment Needs Score. Performs follow-up discharge calls for the hospital and emergency room to ensure continuity of care and timely management of care. Initiates/actively participates in daily huddles, team meetings, and interdisciplinary opportunities to improve patient care at the program level.Promotes patient, family and team interactions and problem-solving byactively participating in interdisciplinary and team meetings to facilitate collaboration and the achievement of identified goals.Documents patient care in the Computerized Patient Care System (CPRS) utilizing the appropriate note title and encounter according to policy and standard operating procedures. Triages acute and non-acute patient needs by telephone, face to face, and secure messaging to facilitate medical management, patient safety, and optimal health. Assists the patient with navigating through the health care systems and intervenes or refers as needed to facilitate the achievement of patient outcomes. Utilizes approved Protocols and Policies to facilitate autonomy in providing care. Applies skills, such as Motivational Interviewing to promote patient self-management towards patient-driven holistic plan for life. Promotes health promotion and population specific care and education. Demonstrates leadership by serving as an advocate for patients, assuming charge nurse responsibilities, and team player to colleagues. Promotes wellness by empowering the patient to self-management through education and patient-driven health care delivery.Involves all members of the teamlet and extended team members in identifying problems, collecting and analyzing data, and problem resolution. Collaborates and develops relationships with supporting services both internally and externally to support patient care needs. Checks daily for outdated medication & supplies to include but not limited to: accucheck controls, Omnicel discrepancies, and crash cart check list. Completes all mandatory training timely and participate in PACT initiatives which includes but is not limited to Secure Messaging, TEACH training, and Motivational Interviewing. Maintains effective working relationships with professionals and support personnel within the medical center and the community. Adheres to principles of infection control, safety, and universal precautions.Initiates/participates in quality improvement opportunities to improve patient care facility wide and at the program level. Enhances patient self-management by providing information regarding all options, choices, and resources.Maintains privacy and confidentiality of all patient information, including electronic, print, and conversations. Coaches and precepts students and nursing staff members. Uses a body of research to validate and/or change work group practice through data-driven evidence-based practices and implement changes to improve efficiency and effectiveness through Systems Redesign approaches.

Pay: Competitive salary, annual performance bonus, regular salary increases

Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
  • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Preferred Experience: BSN and 2 to 3 years current Outpatient Nursing experience.

Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
  • Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
  • Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
  • Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
  • Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
  • Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience.

Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office.

Physical Requirements: Heavy lifting, 45 pounds and over; pushing; reaching above shoulders; use of fingers; use of both hands required; walking 8 hours; standing 8 hours; repeated bending; climbing legs only; both legs required; near vision correctable at 13" to 16" to Jaeger 1 to 4; both eyes required; ability to distinguish basic colors; hearing (aid permitted); emotional stability; far vision correctable in one eye to 20/20 and 20/40 in the other. May be required to work with assaultive or combative patients.
