Duties

Help

Identify candidates for case management and/or care coordination using professional judgment in collaboration with the Primary Care Manager (PCM) and entire health care treatment team.

Collaborates with other healthcare individuals/departments; to include the Health Care Integrator (HCI), Group Practice Manager (GPM), UM/UR, MCSC and other entities in the development of the MTF MM plan in support of the business plan to conduct case management services.

Participates in Population Based Health Care Management Services, applying and promoting wellness and prevention concepts/strategies to the Primary Care assigned patients and those identified for case management.

Provides care for Special Needs Families. Collaborated and coordinates care with a specialized network of providers, MCSC Extended Care Health Option (ECHO) Case Managers, Family Support Center Exceptional Family Member Program Coordinator, UM, MTF providers to ensure a smooth transition and seamless care for families with special needs. Acts as the Special Needs Coordinator (SMC) as needed.

This position will support the Federal Government's response efforts to COVID-19.

Qualifications

This position is notionally projected to transfer to the Defense Health Agency (DHA) within the next 6 months in order to fulfill the congressionally-mandated requirements from the 2017 National Defense Authorization Act, Section 702, as amended in 2019. The incumbent of this position, during such time of transfer, will no longer be a Department of the Air Force civilian and will become a Department of Defense civilian under the authority, direction and control of the DHA. Transfer effective 11 Sep 22.

In order to qualify, you must meet the specialized experience requirements described in the Office of Personnel Management (OPM) Qualification Standards for General Schedule Positions +IOR, GS-0610 series.

INDIVIDUAL OCCUPATIONAL REQUIREMENT:

EDUCATION: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education (external link), Council on Accreditation of Nurse Anesthesia Educational Programs (external link), Accreditation Commission for Midwifery Education (external link), or an accrediting body recognized by the U.S. Department of Education (external link) at the time the degree was obtained. NOTE: You must submit a copy of your transcripts.

Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools (external link) is required for individuals who graduated from foreign nursing schools.

OR

Successful completion of a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field. NOTE: You must submit a copy of your transcripts.

OR

One full year of professional nursing experience (equivalent to the GS-9 grade level) and possession of a diploma, associate degree, bachelor's degree, or master's degree from a professional nursing educational program. NOTE: You must submit a copy of your transcripts.

AND

LICENSURE: Must have passed the National Council Licensure Examination (external link). In addition, must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. NOTE: You must submit a copy of your license.

In addition to meeting the basic requirement above, to qualify for this position you must also meet the qualification requirements listed below:

SPECIALIZED EXPERIENCE: Applicants must have at least one (1) year of specialized experience at the next lower grade GS-09, or equivalent in other pay systems. Examples of specialized experience includes extensive knowledge of nursing care theories, principles, practices, procedures, and extend clinical training and experience, or education, to analyze the full scope of problems associated with providing the best possible care to patients in the most economical manner, and to solve both administrative and professional health care problems; and knowledge of private health insurance policies and procedures to instruct and facilitate acute care thereby improving certification approvals for reimbursement. NOTE: You must submit a copy of your transcripts.

OR

EDUCATION:
Successful completion of a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field; or possession of a diploma, associate degree, bachelor's degree, or master's degree from a professional nursing educational program. NOTE: You must submit a copy of your transcripts.

OR

COMBINATION OF EXPERIENCE AND EDUCATION:
A combination of education and experience may be used to qualify for this position as long as the computed percentage of the requirements is at least 100%. NOTE: You must submit a copy of your transcripts.

FEDERAL TIME-IN-GRADE (TIG) REQUIREMENT FOR GENERAL SCHEDULE (GS) POSITIONS: Merit promotion applicants must meet applicable time-in­-grade requirements to be considered eligible. One year at the GS-09 level is required to meet the time-in-grade requirements for the GS-11 level. TIG applies if you are in a current GS position or held a GS position within the previous 52 weeks. NOTE: Applicants applying as VEOA candidates who are current GS civil service employees or are prior GS civil service employees within the past 52 weeks must also meet time-in-grade requirements.

KNOWLEDGE, SKILLS AND ABILITIES (KSAs): Your qualifications will be evaluated on the basis of your level of knowledge, skills, abilities and/or competencies in the following areas:

1. Knowledge of an extensive range of nursing care theories, principles, practices, procedures, TRICARE concepts, and extended clinical training and experience, or education, to analyze the full scope of problems associated with providing the best possible care to patients in the most economical manner, and to solve both administrative and professional health care problems. Practical knowledge of demand management processes and how these processes are integrated into the total population health.

2. Knowledge of medical facility organization, mission, purpose, philosophy, and objectives as well as those of the American Nursing Association (ANA), The Joint Commission (TJC), Accreditation Association for Ambulatory Health Care (AAAHC) and Health Service Inspection (RSI).

3. Knowledge of private health insurance policies and procedures to instruct and facilitate acute care thereby improving certification approvals for reimbursement. Knowledge of all aspects of disease management to include evidence based primary, secondary, and tertiary prevention protocols as it pertains to case management.

4. Ability to assess patients' needs via critical thinking and diagnostic skills in clinical decision-making, comprehensive health history, physical assessment, health assessment tools, record reviews, and individual patient encounters. Skill in analyzing data defining health care needs of a defined population using standardized assessment tools.

5. Ability to communicate orally and in writing with a diverse group of individual; and to provide guidance to other professional nurses and paraprofessional personnel. Proficiency in computer programs and databases such as Excel, Word, PowerPoint, and department specific programs.

6. Knowledge of a wide range of medical disorders/chronic conditions. Basic knowledge of coding principles and practice.

7. Ability to provide guidance oversight, as well as, directing and reviewing the work of professionals, paraprofessionals and students within the clinic. Ability to provide consultation to other professional nurses and paraprofessional personnel.

PART-TIME OR UNPAID EXPERIENCE: Credit will be given for appropriate unpaid and or part-time work. You must clearly identify the duties and responsibilities in each position held and the total number of hours per week.

VOLUNTEER WORK EXPERIENCE: Refers to paid and unpaid experience, including volunteer work done through National Service Programs (i.e., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; student and social). Volunteer work helps build critical competencies, knowledge and skills that can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.
