The Patient Safety Manager is responsible and accountable for processes/systems which comprise the Patient Safety Program or at the Quality Management service level. Accurately interprets Joint Patient Reporting System (JPSR), follows up with the service and unit level stakeholders to identify opportunities for improvement and tracks and trends this data to provide information and recommendations to the Medical Center Leadership. They conduct regularly scheduled rounds that involve the clinical and non-clinical staff to evaluate, identify, and mitigate identified risks through process improvement and education. Coordinates, aggregates reviews and non-aggregate review Root Cause Analyses (RCAs) when indicated and Healthcare Failure Mode Effects Analyses (HFMEA) in accordance with guidance from the VHA National Patient Safety Improvement Handbook (VHA Handbook 1050.01), VISN, and medical center leadership. Coordinates all VISN QMO/PSO site visits, and NCPS Patient Safety Alerts and Advisories. Conducts ongoing leadership education regarding new/changed patient safety standards. Incumbent mines and analyzes data in order to produce reports, inform services lines and improve performance measures related to patient safety. Involves all members of the health care team in identifying problems, collecting & analyzing data, and resolving care problems of individual or group of Veteran patients. Collaborates with other professionals in identifying patient safety and quality improvement issues and in developing programs and activities to deal with those issues. Collaborates with Education Resource Center Staff to conduct conferences and in-service. Incumbent continually monitors and evaluates the achievement of the Patient Safety Program/Quality Management service goals and objectives and submits an annual evaluation of the patient safety program each fiscal year. Serves as the chair of the Patient Safety Committee and is responsible for developing the agenda and communicating needs and issues that arise through the established governance structure. They initiates and Leads educational activities to promote and enhance clinical understanding, trending and analyzing adverse events and close calls, serving as the organization's expert on the VHA Patient Safety Improvement Handbook.
