Fort Wayne VA Medical Center has served Allen County and surrounding areas. It recently saw a major renovations and upgrades to it Primary care and Inpatient care areas. The Fort Wayne campus has a 26-bed medical center providing acute medical and surgical services. The facility also offers mental health clinics, primary care, medical and surgical specialty care.

The incumbent will serve as a Physician in a Patient Aligned Care Team (PACT) setting. He/she will be responsible for providing medical care in an outpatient setting following PACT principles.

Duties include, but are not limited to:

  • Utilizes all available tools, such as registries, to enable effective and efficient identification and intervention of individual patients and cohorts.
  • Assures that clinical care provided meets all timelines and required documentation for the Chief of Staff (COS) office.
  • Participates in multi-disciplinary team meetings to achieve a patient centered, holistic approach for clinical care and needs of the Veteran, focusing on continuous improvement of critical team processes. Ensures the patient's care plan contains medical recommendations for clinically indicated care.
  • Ongoing, continuous care of one or more assigned panel(s) of Veterans providing health education and health coaching on wellness, disease prevention, chronic care management and self-management skills to patients and personal support persons.
  • Collaborates with informatics technology staff to develop and implement systematized, electronically supported, standardized, tools to support PACT care delivery processes (e.g., pre-visit reminder calls, post hospitalization follow-up calls, recall scheduling procedures, new patient orientation, disease registries and primary care protocols for chronic disease management.

EDRP Authorized: This position may be eligible for the Education Debt Reduction Program (EDRP), a student loan reimbursement program. To participate in the program, you must meet specific individual eligibility requirements in accordance with VHA Policy 1021 and submit your application within four months of appointment. The final award amount (up to $200,000) and eligibility period (one to five years) is determined by the VHA Education Loan Repayment Services office. EDRP approval is contingent on this occupation and specialty being deemed "hard-to-recruit-and-retain" at the duty location you are interested in. Pay: Competitive salary, annual performance bonus, regular salary increases

Qualifications

Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed.
  • Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.
  • Completion of residency training, or its equivalent, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification.
  • Proficiency in spoken and written English.

Physical Requirements: All applicants must meet the physical standards for the position. A physical examination prior to employment may be required. In the process of providing patient care, the work involves regular and recurring periods of walking, standing, sitting, bending, pulling, lifting, reaching and carrying. Manual dexterity is required to operate medical equipment and perform activities such as surgery, treatments and computerized medical record documentation.
