The incumbent serves as a member of the Social Work Service at the Richard L. Roudebush VA Medical Center. The Richard L. Roudebush VA Medical Center is a 1a High Complexity Facility, that serves over 61,000 unique patients and has 13 off site community clinics, in addition to the homeless team, the community based extended care programs and a 50 bed Domiciliary. The incumbent is based in Community Outreach Division.

Help

Duties include, but not limited to:

Administrative Functions:
a. Develops, implements, and maintains all established and new contract programs.
b. Oversees the establishment of program monitors and outcome measures within the assigned program(s) using a data-driven quality assurance process, in, monitoring outcome measures and metrics.
c. Is responsible for Contracting Officer Representative duties, maintaining at least a COR II designation, complies with conflict-of-interest laws, Executive Branch Standards of conduct to avoid conflicts of interest while serving as the CR liaison.
d. Complies with all commonly accepted fiscal practices that are subject to OIG and GAi review.
e. Advocates on behalf of Veterans' service in Homeless Programs before local, state, regional, and federal entities
f. Monitors the clinical care provided by contracted agencies.
g. Provides contracted agencies with necessary technical assistance, when and where appropriate.
h. Reads and enforces the contractor's approved contract and works closely with the contracting officer to ensure compliance.
i. Assesses the compliance of programs receiving CR rate payments and ensures fidelity to contract.
j. Facilitates, develops, and monitors compliance or corrective actions with the contracted agencies.
k. Provides oversight of CR programs as defined in prevailing HPO directives and recommendations.


Clinical Functions:
a. Serves as the Lead liaison (Point of Contact) for the HCHV Programs team for the Indianapolis Medical Center on homeless Veterans in VA ER/hospitalized/inpatient needing Homeless Outreach Services for emergency housing access.
b. Provides clinical care, coordination and case management services by individual or group to Veterans, which includes referral, linkages to various treatment programs within VA and non-VA services.
c. Conducts an admission assessment, clinical reminders, and ongoing screening to inform the contract provider on treatment planning development and changes.
d. Provides crisis intervention.
e. Facilitates and reviews the development of treatment/care plans for Veterans with contract agencies.
f. Completes case notes in VHA health record.
g. Educates Veterans and/or their caregivers about VA and community resources to address their needs.
h. Completes diagnostic tools including the Homeless Operations Management and Evaluation System (HOMES) assessment and HMIS assessment to ensure appropriate housing resources and additional treatment services are received
i. Consults, collaborates, and coordinates with other staff who provide care to Veterans. j. Reviews relevant data, assesses the data, develops ideas, and implements plans for improvement.
k. Participates in the training of medical students, medical and psychiatric residents, nursing students, MSW Trainees, fellows, and other graduate or post-graduate trainees, as appropriate.

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Experience/Education: The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty.
  • Licensure: Must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure
Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs:
  • (a) Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management.
  • (b) Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice.
  • (c) Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes.
  • (d) Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills.
  • (e) Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

Preferred Experience: LCSW: VA Inspection experience, COR certification eligibility

The full performance level of this vacancy is GS12. The actual grade at which an applicant may be selected for this vacancy is GS12.

Physical Requirements: The physical demands of the position involve standard office requirements such as using the telephone, retrieving files, writing reports, data entry and maintenance of computer databases, etc. Work is typically performed in the community (community contract provider, shelters, other community settings). This position requires regular occasional travel. The incumbent must a maintain a valid driver's license and abide by requirements for operating a Government Owned Vehicle. The position requires periods of walking, standing, sitting and bending.
