Total Rewards of a Allied Health Professional
The Clinical Pharmacy Specialist (CPS) will be assigned to Patient Aligned Care Teams (PACT) in the primary care clinics. Teams are located in the main hospital and associated community-based outlying clinics (CBOC). The CPS will be consulted by their team(s) for comprehensive medication management of complex patients with chronic diseases commonly seen in the primary care setting.
The CPS functions at the highest level of clinical practice, works independently under their individual scope of practice to directly care for patients. A CPS functions as a member of their assigned team(s) and serves as an advanced practice provider to initiate, modify, or discontinue medication therapy and as a consultant for comprehensive medication management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. Duties include but not limited to:
Administrative:
a. Completes quality management data compilation for outpatient and inpatient clinical pharmacy activities as required.
b. Serves as a consultant or as a member on various committees as needed and appropriate.
Clinical:
a. Core functions include face-to-face and telephone encounters with referred patients, for comprehensive medication management in practice areas identified in their individual scope of practice.
b. Functions as an Advanced Practice Provider to design, implement, and monitor therapeutic drug plans to achieve definite outcomes through direct interactions with patients and providers in assigned areas.
Indirect Patient Care Activities:
a. Identify patients who may benefit from comprehensive Medication management through population management reports and during team huddles.
b. Review patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes as required.
Scope of Practice:
a. Works under individual Scope of Practice designated based upon the functions of the position. Serves as an Advanced Practice Provider to initiate, modify or discontinue medication therapy.
b. Will participate in a professional practice evaluation program.
Research:
a. Participates in research projects either by acting as a consultant or serving as a co-investigator and participates in the design, conduct and documentation of clinical drug investigations.
Education:
a. Participates in the training and in-servicing of pharmacists, pharmacy technicians, pharmacy students, pharmacy residents, and other staff on assigned teams.
b. Serves as preceptor for pharmacy students and pharmacy residents in accordance with established policies.
Work Schedule: Monday-Friday 0800-1630; weekends, holidays and hours outside TOD (tour of duty) may be required. (Subject to change based upon the needs of the facility).

Pay: Competitive salary and regular salary increases When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).
Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)
