Total Rewards of a Allied Health Professional
The SCC LPN is responsible for observing, recording and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic care plan for life. The LPN demonstrates active collaboration and communication with the Core Team (Patient, Specialty Care Provider, RN Care Coordinator and the Clerical Associate) and the Expanded Team (family/caregiver, internal and community based services involved in providing care to the patient). The LPN must have the ability to react to situations in accordance with established policies and procedures, and ability to appropriately use this knowledge to work effectively with a wide range of patients. Additional functions the SCC LPN will be proficient/independent in include:
a. Utilizing all available tools, such as care management tool, to enable effective and efficient identification, intervention, and follow-up of individual patients and cohorts.
b. Ensuring appropriate evaluation and access is provided to patients assigned to specialty clinics as assigned.
c. Functioning at the full extent of the team member's relevant clinical privileges, credentials, scopes of practice, elements of practice, certification, functional statement, position description, or other VHA or local facility approved documentation of competency.
d. Participating in team performance improvement and sustainment activities to optimize team efficiency and care delivery to patients.
e. Technically proficient in initiating, performing and completing care of the veteran population
f. Recognizes and reports unsafe equipment, conditions, and practice.
g. Actively seeks out educational opportunities to enhance nursing knowledge and skills, and shares with other staff to improve and advance nursing practice.
h. Perform specialty care operations management processes, as appropriate independently with little direct supervision (including the ability to organize work, follow instructions, write and type legibly and accurately).
i. Perform communications and facilitate safe transitions of patients between the SCC site of care and other health care settings, using informal and formal communication methods, as appropriate.
j. Providing health education and health coaching on wellness, disease prevention, chronic care management, and self-management skills to patients and personal support persons, commensurate with the documented expertise or professional training of the SCC member.
Work Schedule: Monday - Friday 10 hour day tours, days vary

Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)

Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.

Physical Requirements and Working Conditions: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required up to 45 pounds. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs.
Receiving Service Credit or Earning Annual (Vacation) Leave: Federal Employees earn annual leave at a rate (4, 6 or 8 hours per pay period) which is based on the number of years they have served as a Federal employee. Selected applicants may qualify for credit toward annual leave accrual, based on prior work experience or military service experience. This credited service can be used in determining the rate at which they earn annual leave. Such credit must be requested and approved prior to the appointment date and is not guaranteed.
