The position is located at the Michael E. DeBakey Veterans Affairs Medical Center (MEDVAMC), and the incumbent performs a variety of duties in this 1A medical facility, classified as the highest in complexity within the Veterans Health Administration. The MEDVAMC provides healthcare services to approximately 123,000 Veterans in Southeast Texas; and outpatient clinics located in Beaumont, Conroe, Tomball, Galveston, Texas City, Lufkin, Lake Jackson, Katy, and Richmond, Texas. The MEDVAMC is an urban tertiary facility with 420 hospital beds, 120 nursing home care beds, and a classified Level 2 emergency room. It is a teaching hospital, providing a full range of inpatient and outpatient services with state-of-the-art technology as well as education and research. Comprehensive healthcare is provided in the areas of medicine, surgery, mental health, spinal cord injury, physical medicine, and rehabilitation, neurology, oncology, audiology, speech pathology, ophthalmology, dentistry, geriatrics, extended care, long-term care, and homecare.
This position is a GS12 Social Worker position that is assigned to the Domiciliary Residential Rehabilitation and Treatment Program (DRRTP). The incumbent must use a high level of skill in assessing and treating the complicated psychosocial problems of Veterans. Care management responsibilities include the management, coordination, and provision of social work and supportive services to Veterans and/or their families. The incumbent is a member of an interdisciplinary team that thoroughly assesses the needs of Veterans in order to plan and provide a therapeutic environment that will assist the Veteran in rehabilitation of optimal levels of medical and psychological health, chemical abstinence, vocational assistance, and securing benefits needed to reintegrate to community living.
The incumbent is responsible for coordinating the day-to-day operation and management of applications and admissions to the MHRRTP programs, to including managing a patient pending waiting list if needed to be entered into the Bed Management System (BMS).
The incumbent works with members of an interdisciplinary team in responding to consults for MHRRTP admissions. This requires familiarity with eligibility requirements. MHRRTP admission applications must be reviewed by a designated screening and admissions process composed of either a MHRRTP interdisciplinary team or designated MHRRTP-based admission coordinator. Veterans must be admitted according to the Veteran's needs for services and the MHRRTP's ability to provide those services.
