The Post 9/11 Military2VA Program Manager is a Social Work Program Coordinator whose primary responsibilities are to ensure that ill and injured service members and veterans receive patient-centered, integrated care. The Program Manager has administrative and clinical responsibility for the M2VA program. The incumbent spends at least 25% of their time in administrative coordination of this program. There is also an expectation maintaining a clinical caseload. The incumbent develops policies and procedures for program operation and prepares reports and statistics for facility, VISN, and national use. The incumbent may be responsible for the program's budget and various control points assigned to the program within the assigned areas. The incumbent manages and controls the use of data related to these programs regarding day-to-day functioning and program evaluation. In conjunction with the Social Work Executive, the incumbent plans, organizes and directs administrative, operational, and Veteran and personnel activities for the service.
Duties include, but are not limited to:
- Leading the Post 9/11 M2VA team at the Health Care facility which includes M2VA case manager(s); ensuring the M2VA policies and procedures are in place and facilitating communication among the M2VA team.
- Overseeing all Health Care case management activities provided to transitioning, ill, or injured service members and veterans.
- Ensuring that all transitioning, ill, or injured service members and veterans are screened to determine need for nurse and/or social work case management services and complex care coordination.
- Collaborate with Care Coordination and Integrated Case Management (CCICM) to ensuring that all severely ill or injured transitioning service members or veterans and other new service members and veterans in need of case management are assigned a Lead Coordinator (LC).
- Participate in CCICM Care Coordination Review Team (CCRT) to review new and/or established patients' clinical status and appropriate designation of a LC.
- Providing ongoing communication and periodic briefings to senior Health Care Facility Leadership regarding the Post 9/11 M2VA program.
- Developing and maintaining effective partnerships throughout the facility and satellite clinics.
- Acting as a back-up team member in the absence of the case manager or arranging for team member coverage when appropriate.
- Serving as the primary point of contact at the facility for the VHA DoD Liaisons for Healthcare and the Military Treatment Facility (MTF).
- Developing a business plan that outlines the program's mission, strategic goals and objectives, forecast the yearly budget, and provides and appropriate resources for achieving stated outcomes.
- Developing an orientation and continuing education plan for new and existing M2VA staff.
Work Schedule: Monday-Friday, 7:30 a.m. - 4:00 p.m. or 8:00 a.m. - 4:30 p.m.

Pay: Competitive salary and regular salary increases.

Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
