The Grant Per Diem Liaison/Case Manager is assigned to administrative responsibility for clinical program development and is accountable for clinical program effectiveness and modification of service patterns.
Assigned to work with special patient populations with highly complex health or mental health problems, such as VA/DoD liaisons assigned to Military Treatment Facilities, or social work case managers in Mental Health Intensive Case Management (MHICM) programs.
Implementation of VA's Health Care for Homeless Veterans (HCHV) Programs, Homeless Providers Grant and Per Diem (GPD) Program is authorized by Public Law 107-95. The law authorizes the Department of Veterans Affairs to assist public or non-profit private organizations in establishing new programs by awarding grants, through a competitive process, for up to 65 percent of the cost of acquiring, renovating, or constructing facilities. The law also authorizes VA to provide per diem payments to grant recipients (or non-grant recipients through special funding) that provide supportive services and/or supportive housing for homeless Veterans.
Provides case management at Homeless Grant and Per Diem Program sites. Utilizes various treatment modalities of individual and group work, casework, community organization, and community outreach activities to alleviate or remove problems that interfere with the Homeless Grant and Per Diem Program participants' ability to achieve independence.
Monitors the clinical care and assesses the compliance of the programs receiving per diem as described in the grant recipient's original grant application.
Provides services and oversight of the Homeless Grant and Per Diem funded programs including verification of Veteran status and eligibility for participation in the Homeless Grant and Per Diem Program, maintaining admission and discharge dates of participants for billing purposes, reviewing and approving/disapproving readmissions to and extensions of care and collecting and submitting participant information as outlined by NEPEC evaluations procedures.
Coordinates and participates in program site inspections utilizing the Grant and Per Diem Program rules, regulations, and directives as guidance and forwards completed inspection packages to the appropriate offices.
Coordinates care including serving as an advocate on behalf of the Veterans to ensure comprehensive service delivery and linking and referring Veterans to the VA Medical Center, VA Regional Offices and/or community agencies.
Engages in coordination with community agencies, organizations, and groups to learn of services or resources available for homeless Veterans as well as provides current information on services and benefits available in the VA for Homeless Grant and Per Diem Program participants.
Participates in program planning and development, advising the supervisor of shifting trends with caseload and identifying the need for and making recommendations on program procedure or policy changes based upon work experiences.
Works with VA Medical Center multi-disciplinary teams and community agencies to develop more effective and efficient ways of delivering healthcare services to Homeless Grant and Per Diem Program Veterans.

Pay: Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).
Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year). Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
