General Description of Assigned Duties:
Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Serves in an administrative role as a Primary Care Patient Aligned Care Team (PACT) champion and coordinates local PACT implementation, training, communication, and sustainability of the PACT model. Actively promotes the PACT model within the Ambulatory Care setting, educates core PACT team members, identifies, and addresses communication and workflow challenges, collaborates with leadership and associated stakeholders, and schedules meetings among teams. Influences care outcomes by collaborating with members of the interdisciplinary team. Core elements of performance include knowledge and active participation in the unit/program level quality improvement process and initiatives as well as customer service programs.
Functions or Scope of Assigned Duties:
The Nurse II PACT Coordinator is responsible for providing competent, evidence-based practices. Demonstrates leadership in delivering and improving holistic care through collaborative strategies with others. Questions clinical practices for the purpose of providing evidence-based care. Demonstrates the ability to cope with and manage competing priorities. Evaluates practice in an ongoing process, based on best evidence. Provides peers with informal constructive feedback for improvement. Fosters a safe and supportive environment conducive to the professional development of healthcare professionals. Contributes professional nursing perspective in discussions with the interdisciplinary team. Partners with others to effect change and produce optimal outcomes. Executes position to promote full PACT implementation and function within the Primary Care setting. Focuses on the implementation and sustainment of the PACT model driven by quality improvement and evidence-based decision-making. Interacts with and assists PACT members in achieving compliance with Veterans Affairs Central Office (VACO), Veteran Integrated Services Network (VISN), and local policies and directives. Assists teams to maintain national Primary Care standards using clinical practice guidelines and clinical pathways. Promotes PACT fundamentals through staff education, clinical coaching, and quality improvement and metric monitoring. Conducts knowledge sharing to provide safe, efficient, quality PACT care. Fosters a safe and supportive environment of care conducive to the professional development of PACT and extended PACT healthcare professionals.
The Float Patient Aligned Care Team Coordinator will be required to float to varying clinics throughout Eastern Colorado Health Care System as needed based on department, facility, or patient care needs.
Supervisory Controls:
The incumbent is responsible to the assigned rating and approving supervisors and is accountable to the Associate Director of Patient Care Services (ADPCS).
