The Home Based Primary Care physician is responsible for the overall quality of medical care and treatment to the Veteran population as assigned. The physician serves as the primary point of accountability in Geriatrics and Extended Care, including Home Based primary Care and Community Living Centers, and maximizes the efficient use of available resources to treat assigned population(s).
