The Compensated Work Therapy CWT Transition Residence (TR) and Transitional Work (TW) Outreach Coordinator works at the San Francisco VA Health Care System Downtown CBOC, independently coordinates the CWT Transitional Work (TW) and CWT Transitional Residence (TR) Programs. The CWT TR/TW Outreach Coordinator provides outreach to homeless or marginally housed Veterans who underutilize VA services. The mission of the position is to identify and engage Veterans who are interested in vocational rehabilitation services, would benefit from a transitional living experience, or are experiencing homelessness/marginally housed and would benefit from outreach and linkage to the San Francisco VA Health Care System.
The CWT TR/TW Outreach Coordinator provides Veterans with a range of social work services, including assessment and referrals, clinical treatment, short-term case management, advocacy and service coordination, and linkage with VA and community services. This is accomplished in concert with other members of the Downtown CBOC multidisciplinary team and community partners. The CWT TR/TW Outreach Coordinator possesses knowledge and is sensitive to the vocational/employment needs of Veterans hoping to return to the workforce, and marginally housed Veterans, and has working knowledge and experience in medical, mental health, and substance abuse treatment modalities. The CWT TR/TW Outreach Coordinator is able to complete a comprehensive psychosocial assessment to identify strengths, needs, social support systems, disabilities, and barriers to accessing housing and available treatment options. The CWT TR/TW Outreach Coordinator has general knowledge of Veterans' benefits and services related to special programs, service-connected compensation, non-service connected pensions and benefits that are available on the federal and state levels and through city/county social services.
The CWT TR/TW Outreach Coordinator possesses knowledge of work hardening skills and the philosophy of preparing Veterans for re-entry into regular employment or are not engaged with VA services and require outreach. The CWT TR/TW Outreach Coordinator is sensitive to the needs of Veterans and is aware of the unique challenges in working with Veterans with co-morbid physical, mental health and substance use disorders. The CWT TR/TW Outreach Coordinator must possess a valid California driver's license to be able to drive between the houses and might transport Veterans on activities such as social outings. The CWT TR/TW Outreach Coordinator's work site, in addition to the Downtown CBOC, is the Medical Center's CWT Transitional Residences - located on Lombard Street and Burnett Avenue. Duties also include participating as a member of the Health Care for Homeless Veterans (HCHV) team, which includes assisting with screening, admissions and case management needs for Veterans presenting at the San Francisco Downtown CBOC.
