This announcement will be used to fill multiple vacancies across multiple services. Please not that not all Service are at all listed duty stations. The Services that are associated with this announcement are:
Primary Care - The Licensed Practical Nurse is assigned to Ambulatory Care Service provides technical nursing care to adult and geriatric patients according to a plan of care established by a Registered Nurse and Patient Aligned Care Team (PACT) requirements.
Community Living Center - The Licensed Practical Nurse functions under the direct supervision of the Registered Nurse who is responsible for the coordination of resident care. The LPN works with the RN and others to appropriately orient less experienced LPN's, Nursing Assistants (NAs) or Health Care Technicians (HCT).
Medical Service - The Licensed Practical Nurse demonstrates active collaboration and communication with the Outpatient Medicine Team (patient, specialty care provider, and clerk) and Expanded Team (family/ caregiver, internal and community based services) involved in providing care to the patient.
Physical Medicine & Rehabilitation Service - The Licensed Practical Nurse provides nursing care to chronically and/or acutely ill patients on a regular and recurring basis under the supervision of the RN, Nurse Practitioner, and/or the physician.
Major Duties include but are not limited to:
- Prepares, administers, records, interprets, and communicates results of medications, vital signs, treatments, etc.
- Appropriately reports and records gross and subtle changes in patient's status.
- Under minimal supervision of the RN, functions as the primary nurse and gives the change of shift report to the oncoming shift.
- Recognizes and initiates appropriate response in emergent situations such as fire, disaster, bomb and/or hostage threats, cardiac and/or pulmonary arrests, and behavioral emergencies.
- Carries out emergency measures and initiating procedures for obtaining medical/other types of assistance. Initiates CPR and inserts an oral airway as needed.
- Completes pain evaluations, and on the basis of pre-formulated guidelines, either provides education, or refers for further evaluation.
- Adheres to asepsis and universal precautions requirements to provide a safe environment for patients, families, and employees. Sets up and cares for patients in isolation per infection control standards.
- Contributes to the interdisciplinary team in development, implementation and evaluation of the patient's treatment plan and promotes patient participation in achieving established goals by fostering self-reliance and independence. Documents as appropriate
