The Tomah VA Medical Center, Tomah, WI is recruiting for an experienced and highly qualified Board Certified/Board Eligible physician for Geriatrics and Extended Care for inpatient care, graduate medical education and scientific research as appropriate. This position will provide oversight to Geriatric Medicine in the GEC Department. The incumbent will provide direct patient care, offer support to the nurse practitioners and/or physician assistants and work with additional interdisciplinary staff. He/she will report directly to the Associate Chief of Staff (ACOS)-Medicine Services (MS). The ACOS-MS is the supervisor of this position. The physician in GEC will act as a point of contact for fellow physicians and clinical staff assigned to the Department. This will include, but is not limited to, documentation, process workflow and other functionality regarding necessary clinical systems. This individual will work with staff and physicians to optimize the usage of clinical systems to promote quality outcomes and providing support in accordance with organizational goals. He/she will work closely with hospital staff, department directors, and physicians to ensure that the highest standards of quality and services are maintained. He/she also serves as a liaison between customers, Information Technology staff and other departments and services. This individual will also work directly with all levels of the organization on projects, committees and teams to meet the overall needs of Geriatrics and Extended Care and the facility. The Physician, Geriatrics and Extended Care, should possess clinical experience, be familiar with a variety of the field's concepts, practices, and procedures and possess experience and judgment to plan and accomplish goals. It is preferred that the incumbent is board certified in Geriatric Medicine. This individual will perform a variety of complex tasks and will lead and direct work of others. The Physician, Geriatrics and Extended Care, will also perform as a backup provider for the Hospitalist / Medical Officer of the Day (MOD) and placed in a standard rotation for coverage when needed. This job requires knowledge of administrative procedures, quality measurement, regulations related to the practice of medicine, Long-term Care Standards, Joint Commission, and other regulatory standards pertinent to Geriatrics and Extended Care.
- Admit, follow and discharge Community Living Center (CLC) patients as needed to maintain good patient flow
- Provide geriatric and palliative care consultation for the in-patient population. Provide clear, complete direct and electronic communication with staff in all areas of the Medical Center and with community contacts to facilitate smooth patient transitions, quality patient care and high customer satisfaction. Participate in interdisciplinary team meeting (IDT) weekly
- Collaborate with the GEC nurse practitioners/physician assistants, including case reviews and preceptor reviews
- Participate in care plan meetings and family meetings in conjunction with clients' care needs- Attend and practice in Geriatrics and Life Sustaining Treatment Decision Clinic
- Meet ethical and professional standards as written in the Medical Staff Bylaws and ensure that work products are in compliance with written and communicated facility expectations, standards, policies and procedures
- Provide informal clinical consultation for the GEC staff- Participate in selected VA committees as needed
- Maintain current requirements for credentialing and privileging including necessary license and certifications
- Pursue high quality continuing education and professional development opportunities
- Participate in appropriate regional and national professional organizations
- The healthcare field is an ever-changing and requires constant updating of knowledge. It is therefore important that the incumbent participate in continuing education that will enhance health care management skills and knowledge
- Duties require numerous daily contacts with patients, employees and visitors, in addition to VA headquarters representatives, VISN representatives, medical care representatives, officials of other government agencies, representatives of our educational affiliates, community and State health care providers, etc.
