Functions as the Physician, Patient Aligned Care Team (PACT), Primary Care Services, Community Based Outpatient Clinic (CBOC) setting at Montana VA Healthcare System (MT VA HCS). Responsible for providing medical care in an outpatient setting following PACT principles. May provide medical care using Telehealth for remote patients or travel to other CBOCs to cover provider shortage. Participates in multi-disciplinary team meetings to achieve a patient centered, holistic approach for clinical care and needs of the Veteran, focusing on continuous improvement of critical team processes. Ensures the patient's care plan contains medical recommendations for clinically indicated care. Ongoing, continuous care of one or more assigned panel(s) of Veterans providing health education and health coaching on wellness, disease prevention, chronic care management and self-management skills to patients and personal support persons. The incumbent must be able to work collaboratively with ancillary staff including nursing and clerical staff and on-site Mental Health Providers.
Work Schedule: Monday - Friday; 8:00am to 4:30pm or as agreed upon with possibility of extended hours and weekends. NOTE: Candidates must be willing to work weekends, irregular work hours and extended hours. The candidate must be willing to be assigned to other related services and/or locations, if necessary.

Montana VA has served veterans since the end of World War I, when Fort William Henry Harrison became a Public Health Service hospital and then VA medical facility, treating soldiers with tuberculosis caused by mustard gas and other chemicals and fumes. Today, half of the almost 100,000 Veterans that call Montana home, are enrolled in the Montana VA Health Care System, which provides services in every major city in the state through eleven community based clinics, two telehealth outreach clinics, the Fort Harrison VA Medical Center (VAMC), the Billings Health Care Center, and the Miles City Community Living Center. Fort Harrison VAMC, just outside of Helena, is a 24-bed acute care, medical-surgical facility that offers a broad range of acute, chronic, and specialized inpatient, outpatient, and telemedicine services for both male and female Veterans. The 24-bed Behavioral Health facility provides services for patients needing treatment for PTSD and Substance Abuse.
