The Licensed Vocational Nurse (LVN) works under the direction of a Registered Nurse (RN) and reports to the Nurse Manager for the East Side Community Based Outpatient Clinic (CBOC). The LVN is responsible for observing, recording and reporting changes in the patient's condition. The LVN requires a general review of their work by an RN for appropriateness and conformity with license, policies and procedures.

The LVN is an advocate for the patient and the patient's family and promotes safety by practicing within their scope of practice and in accordance with local policy and procedures. LVN scope of practice does not include acts of medical diagnosis or the prescription of therapeutic or corrective measures. The practice of vocational nursing must be performed under the supervision of an RN, APRN, physician, physician assistant, podiatrist or dentist. Supervision is defined as the active process of directing, guiding and influencing the outcome of an individual's performance of an activity. The LVN is precluded from practicing in a completely independent manner; however , direct and on-site supervision may not be required in all settings or patient care situations.
The LVN is responsible for the daily operations in support of a primary care provider witht he following duties: Patient screening with vital signs and reason for seeking care documented, completion of clinical reminders, medication administration via oral, intramuscular and topical routes, immunizations, patient calls, routing and follow up of patient messages, as well as dressing chagnes and assisting providers with invasisve procedures such as joint injections. Each assigned LVN will conduct blood pressure and glucose checks as ordered by provider staff. All Clinical documentation is entered in the electronic patient record with strict security protocols followed.
