The Nurse is responsible for care coordination of services internal and external to the VA to facilitate care transition in order to effectively meet the patient needs according to established standards of care/practice, policy and procedure. Provides patient and family health education, care coordination and direct patient care.
Demonstrates proficiency in practice based on conscious, deliberate planning. Self-directed in goal setting for managing complex patient situations (VA Handbook 5005, Part 11, Appendix G6). The Registered nurse is a competent nurse who demonstrates competence in practice and decision-making, deliberate planning, and critical thinking skills. This nurse integrates knowledge, skills, abilities, and judgment and is self-directed in goal setting for managing complex situations. The focus is beyond the self to the evolution of systems thinking by synthesizing knowledge to solve problems that result in demonstrated outcomes. This nurse demonstrates the ability to cope with and manage competing priorities. The competent nurse evaluates others, coaches and mentors others, and demonstrates leadership skills through collaborative efforts.
The registered nurse provides care and counseling of persons or in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, health counseling; provision of care supportive to or restorative of life and well-being; and executing medical regimens including administering medications and treatments prescribed by a licensed or otherwise legally authorized provider (Wyoming Board of Nursing). The nurse's primary commitment is to the patient, whether an individual, family, group, or community (ANA Code of Ethics for Nurses).
Depending on which duty station the incumbent is selected for, the incumbent will be responsible to either the Loveland, Multi Specialty Based Outpatient Clinic (MSOC) Nurse Manager or other Nursing Management, OR the Fort Collins Multi Specialty Based Outpatient Clinic (MSOC) Nurse Manager, with practice accountability to the Associate Director Patient Care Services/Nurse Executive.
Work Schedule: Various Shifts and Days, Monday through Friday, 0700-1730
