Incumbent is a Clinical Social Worker assigned to VA Northern Indiana Health Care System's Social Work Service, performing duties as the Mental Health Case Management (MHCM) Program Coordinator under the program direction of Mental Health Service . The Mental Health Case Management (MHCM) Program serves veterans with severe chronic mental illness. Services are recovery-oriented and are primarily delivered to veterans in the community. These services are delivered by an integrated, multidisciplinary team with a focus on a high intensity of care, crisis prevention and management, flexibility, and community-oriented setting. Further, MHCM focuses on rehabilitation through practical problem solving, crisis resolution, and adaptive skills building with transition to self-care and independent living where possible.
Veterans enrolled in this program have severe and persistent mental illness. They have a severe functional impairment such that they are unable to maintain a stable community living situation or participate in necessary treatments without intensive support. They are often inadequately served or unable to achieve stable community adjustment with conventional clinic-based outpatient treatment. They have high hospital and emergency room utilization.
The role of the MHCM Team Leader is to develop, direct and coordinate the intensive case management services that are provided by the MHCM team to veterans in the community. The role requires incumbent to conduct, evaluate, and coordinate the execution of the program (this may includes coordination of the following activities: day-to-day staffing schedules, staff productivity, program/staff efficiency, program/staffing cost effectiveness, quality performance, outcomes and/or coordination of clinic effectiveness and clinic productivity). The incumbent is expected to conduct program analysis, provide appropriate assessments of the outcomes and make recommendations as needed.
The MHCM Team Leader provides 50% administrative/team leadership and 50% clinical duties.
