The Licensed Practical Nurse (LPN) in the Ambulatory Care Clinic is an integral part of the Patient Aligned Care Team (PACT). The PACT LPN works under the direction of an RN and reports directly to the Ambulatory Care Nurse Manager. The LPN is responsible for observing, interviewing, gathering data, recording and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic care plan for life.

The LPN requires a general review of their work by a RN for appropriateness and conformity with license, policies and procedures. The LPN demonstrates active collaboration and communication with the PACT Team (Patient, Primary Care Provider, RN Care Manager and Medical Service Administrative Assistant (MSA) and larger team (family/care giver, internal and community based services and disciplines involved in providing care to the patient). The LPN will provide prescribed medical treatment and personal care services to ill and injured Veterans. Data entry is a large portion of the daily work assignment.
Work Schedule: This position is full-time, and the hours of duty will vary between 6:00 am - 6:00 pm, Monday through Friday (8 hour shifts).
Primary Duty Station: This announcement is for the Fort Knox Community Based Out Patient Clinics, and may be required float between all of the Ambulatory Care Community Based Outpatient Clinics (CBOC) based on the staffing needs of the service.
