The RN Community Care Program Manager is the clinical and administrative coordinator for all aspects of Non VA Care and Care Coordination including policies, practice, budgetary, and administrative oversight of compliance with VHA Directives in the Community Care Program. Provides oversight for interdisciplinary team members, reviews and approves Non VA Care consult requests for medical necessity, and reviews and approves or denies Non VA care claims based on eligibility criteria. Establish and maintain effective relationships with all levels of medical center personnel and relate effectively with customers, families, and the community as well as program and service leaders at the local and network levels. Assist with policy making activities as related to customer service and the overall functioning of administrative and clinical programs. Collaborate with other health care staff to establish and maintain programs that cross service and/or discipline lines and influence organizational mission and health care. Provide executive leadership and direct supervision for the Community Care Program and oversight of all functions associated with traditional Non VA care, purchased care, provider agreements, financial, and regulatory controls. Provide administrative, clinical, and financial oversight to purchased services including homemaker home health, skilled nursing, hospice, and inpatient care; outpatient care including scheduling, authorizations, provider agreements, reconciliation of budget and paid claims; case management of patients referred to the community, traveling Veterans and hospital notifications. Incumbent will also be a liaison with VA providers, network providers, patients, and staff. Will also be required to monitor progress toward goals using multiple data sources. Position may entail other duties as assigned.
Work Schedule: Days 8:00 am to 4:30 pm (hours may vary based on facility needs)
