The Community Health Nurse provides patient and family health education with a focus on self-management, prevention, and wellness based on the patient's goals. Demonstrates advanced leadership skill by providing coordination and oversight of complex specialized community healthcare placement. She/he serves as a preceptor or mentor to colleagues or students to enhance professional growth, development and practice. She/he is responsible for demonstrating and maintaining mastery clinical expertise and knowledge of health care programs to improve the quality of patient care. She/he participates in quality improvement activities at the unit, service and Medical Center level with the objective of improving patient care outcomes. The Community Health Nurse participates in Social Work Service and Medical Center Committees, representing nursing as assigned. She/he takes an active role in identifying his/her own education needs and continually updates clinical skills. The incumbent reports directly to the Social Work Service Supervisor of Home and Community Based Services.
(a) Leading and organizing delivery of care to assure continuity of care and peer accountability for practice, including access to care and discharge planning
(b) Using advanced clinical knowledge/judgment to promote staff involvement in planning, decision-making, and evaluating outcomes
(c) Functioning as an expert in clinical practice and/or areas related to the assigned roles and responsibilities
(d) Systematically evaluating current practice, and formulating outcomes for groups of patients and/or organizational processes within an area of expertise
(e) Guiding, developing, and supporting staff from a leadership perspective, and
(f) Using professional standards of care, scientific evidence, and practice to evaluate programs and/or service activities.
Work Schedule: 8:00-4:30 Monday-Friday; Schedule may be changed to meet the needs of the Veterans and the medical center during emergent situations; and evening and weekend coverage may be required.
