The Registered Nurse Care Manager is responsible and accountable for all elements of the nursing process when providing direct Veteran care in the ambulatory primary care (Patient Aligned Care Team) environment uses the nursing process and evidence based practice to perform nursing functions related to coordination of patient care utilizing patient centered practices, and a focus on excellence in nursing care and customer service. Coordination of care includes the facilitation of shared decision making, identification of patient care needs, facilitation of patient identified goals for care using motivational interviewing skills, development and modification of a patient specific plan of care, monitoring of patient outcomes, coordination of services between VA facilities and non-VA facilities, facilitation and utilization of established chronic disease management processes, and coordination of transitional care after discharge. She/he collaborates with patients to assess and identify needs, issues, care goals, and resources in critical factors for achieving desired outcomes related to discharge, post hospital recovery and health maintenance/wellness. The Registered Nurse Care Manager utilizes patient information databases to identify at-risk Veterans requiring coordination of care, utilization of and participation in shared medical appointments, and use of decision support tools along with critical thinking skills to triage Veterans with symptom related complaints. The Registered Nurse Care Manager will establish patient education goals and establish a nurse run follow-up clinic to obtain positive patient outcomes.
The Registered Nurse Care Manager collaborates with other team members and the larger team (specialty care, family/caregiver, internal and community-based services involved in providing care to the patient) in developing the patient-centered and holistic care plan. The Registered Nurse Care Manager monitors and tracks consults in collaboration with the primary care team. Assists in ensuring the Veteran receives the right care, at the right time, in the right place.
Work Schedule: 0730-1600 or 0800-1630, Monday through Friday
