The Licensed Practical Nurse is a direct caregiver who has the knowledge and skills necessary for concentrated effort and self-direction in carrying out complex nursing care assignments and who completes duties and responsibilities, which involve performing nonstandard and specialized practical nursing activities requiring experience and demonstrated skill sufficient to resolve a range of nursing problems. Work is reviewed in collaboration with the Ambulatory Care Nurse Manger and the Women Veterans Program Manager, however the LPN works independently and requires little supervision by the RN.
The Licensed Practical Nurse serves as the Women's Health Care Coordinator under the direction of the Women's Program manager and reports to the Nurse Manager who ensures that work is complete and accurate. The LPN is responsible for observing, recording and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic care plan for life. The LPN is a member of the larger Team. The LPN requires a review of their work by an RN/provider for appropriateness and conformity with license, policies and procedures. The LPN demonstrates active collaboration and communication with the Women Veterans Program Manager, Utilization Management, Health Administration, and Radiology team members. Also, this will include as well as the Teamlet (Patient, Primary Care Provider, RN Care Manager and Clinical Support/ Patient Service Administrative Assistant (PSA) and Larger Team .
Work Schedule: 7:30am-4:00pm, Monday thru Friday with occasional extended hours and Saturday morning clinics.
