Works collaboratively with other healthcare providers throughout the VANCHCS via Telehealth (Live/Store-and-Forward) and Voice/Electronic (email/IM) communication. The Registered Nurse for Wound/Ostomy is responsible for collecting and communicating data reports for the tracking of HAPUs/quality care indicators. The Registered Nurse (RN) for Wound/Ostomy Care provides direct patient care for the Outpatient in the Nurse-run Wound/Ostomy Clinic. The RN will be responsible for reviewing consults, triage and schedule of patients; telephone visits addressing questions/concerns including prescription renewal; scheduling follow-up and as needed encounters; address walk-in and on-demand concerns; maintain huddle-board/start of shift team communication; contribute to monthly quality improvement meetings such as All-Staff, Multidisciplinary, Nurse, and Wound/Ostomy/Continence (WOC) specific groups; maintain inventory of clinic stock to include replenishment from pharmacy and warehouse; review product selection; meet with vendors; coordinate in-service of new product; collaborate with Podiatry/Dermatology in a multidisciplinary wound clinic 1 day/week; work collaboratively with multidisciplinary wound clinic on active research projects; precept WOC students; precept/host nurse and physician residents; supervise/mentor high school ROP students; provide continuing education for employees; provide LVN/CNA/MA oversight; chart review; care plan development. The Registered Nurse for Wound/Ostomy is responsible for maintaining certification and continuing education for Wound/Ostomy specialties.
The Wound/Ostomy RN will demonstrate proficiency in risk assessment; wound/ostomy assessment; topical management of wounds; product selection for ostomy patients; nutritional assessment as related to wound and ostomy care; pressure ulcer staging; management of pressure injuries, neuropathic/diabetic ulcers, vascular/arterial ulcers, and palliative wounds/skin breakdown. Will maintain proficiency in wound/ostomy documentation and interpersonal communication.
